35 hours per week (full-time)
flexible over Monday to Friday, 8:30am to 5:00pm.
Salary FTE: £25,806 per annum
Term: Permanent
Join our friendly ‘Going Home from Hospital’ team to support people to return home from hospital by listening to their needs, making referrals and connecting them to the opportunities and support available. We are looking for someone who enjoys team working, who has great interpersonal skills and the ability to problem solve, often in ‘real time’. Ideally, you’ll already have in-depth knowledge of the services offered by the voluntary sector, but if not you will be supported to learn. You’ll be working with other organisations in the health, social care and voluntary sectors to enable people to return home safely and on time with the support they need to live independently. This role is all about team work, for the benefit of individuals, in a fast-paced environment. If you like working with people, are a good listener, effective communicator and know how to build relationships with people, we’d love to hear from you. We are looking for someone with the drive and commitment to help us make a difference.
You will mainly work from either Southmead Hospital or the BRI, but may occasionally be required to visit other locations such as City Hall. There is some opportunity for working from home on admin tasks. This is what the current postholder says:

You will be working with a cross-sector team of professionals within the hospital and supported by a friendly team of people who are based at Age UK Bristol’s office at The Withywood Centre, Queens Road, BS13 8QA.
