Lifestyle Client Assessor

16 hours per week - £22, 604 pa (pro rata) - plus pension and excellent holidays

We’re looking for a compassionate and organised Lifestyle Client Assessor to carry out
home-based assessments for people aged 50+ who wish to access our Lifestyle Service.
You’ll identify support needs, complete risk assessments, and pass accurate information to
the Lifestyle Team to ensure the right support is provided. This is a rewarding role where
you’ll make a real difference to the lives of older people across Sunderland, helping them to
remain independent, safe, and supported in their own homes.

As a Lifestyle Client Assessor, you will be the first point of contact for individuals aged 50+
who wish to access our Lifestyle Service. You will carry out home visits, complete thorough
support and risk assessments, and identify what help each client needs — such as light
housework, shopping, or companionship. Your findings will then support the wider
Lifestyle Team in matching clients with the right Lifestyle Worker.

Candidates are required to have a full clean driving licence and have access to a car for work.
