Lead a new approach to improving health for people with learning disabilities
Hamelin is launching an ambitious pilot across Billericay and Tendring to help tackle one of the biggest health inequalities in the UK - the gap in health outcomes for people with learning disabilities.
We’re looking for a passionate Learning Disability Nurse to lead this work alongside local GP practices and wider community organisations.
About the role
This is not a traditional nursing role.
Working alongside our Director of Care and another Learning Disability Nurse, you’ll design and lead a new model of support, helping people understand their health, co-create meaningful Health Action Plans (HAPs), and achieve real improvements in their wellbeing.
Working closely with GP practices, you will:
- Support increased uptake of Annual Health Checks (AHCs)
- Ensure every patient receives a personalised, accessible HAP
- Help individuals take practical steps to improve their health (e.g. weight, smoking, activity, social connection)
- Build strong partnerships across health, social care and the voluntary sector
- Recruit and line manage a Health Care Assistant to support delivery
What you’ll be working towards
This pilot has clear, measurable outcomes. You will play a key role in delivering:
- Increase AHC uptake from current levels to at least 50%
- Ensure all patients on participating LD registers are offered a Health Action Plan
- Co-create at least 50 in-depth, personalised HAPs
- Achieve improved health outcomes for at least 80% of participants
- Deliver education sessions for people with learning disabilities and their families
- Improve confidence and capability within GP practices to deliver accessible care
- Contribute to reducing long-term health inequalities and avoidable deaths
You’ll also contribute to the development of a future vaccination programme, helping ensure people with learning disabilities can access immunisations in a way that works for them.
What we’re looking for
If you’re someone who:
- Wants to drive real system change
- Believes care should be accessible, personalised and empowering
- Enjoys building relationships and leading innovation
- Is confident working across organisations and influencing practice
…we would love to hear from you.
What we offer
- A unique opportunity to lead an innovative, high-impact pilot
- Flexible and supportive working environment
- Blue Light Card
- Cash Health Plan
- Employee Assistance Programme
- Training and development opportunities
- Pension
Why this role matters
People with learning disabilities are significantly more likely to experience poor health and avoidable deaths.
This role is about changing that - ensuring people not only receive health checks, but are supported to understand and act on them in a way that works for them.
