Looking for a role where you can genuinely change lives?

We're looking for a passionate Health and Wellbeing Practitioner to join our team at HMP The Mount, supporting individuals on their journey towards recovery, better mental health, and a positive future beyond custody.

This is a full-time position (35 hours per week) offering a starting salary of £24,000. A salary of up to £25,844 may be offered to candidates who can demonstrate relevant experience.

In this rewarding and varied role, you’ll deliver person-centred support using both harm reduction and abstinence-based approaches. Working as part of an integrated healthcare team, you’ll support individuals experiencing substance misuse issues, low-level mental health needs, and wider health and wellbeing challenges, helping them build resilience and prepare for release back into the community.

What you’ll be doing

You’ll play a key role in delivering high quality interventions and support, including:

  • Managing a caseload and delivering personalised support and care plans
  • Completing assessments, risk reviews and structured interventions
  • Delivering 1:1 sessions and group programmes on substance misuse and low-level mental health topics
  • Providing harm reduction advice and health and wellbeing guidance
  • Supporting service users with release planning and continuity of care into the community
  • Working collaboratively with healthcare, probation, housing, employment and external recovery services
  • Maintaining accurate case records, reports and outcome data
  • Contributing to service performance targets and quality standards

What we’re looking for

We’re looking for someone who is compassionate, organised and motivated, with the ability to build professional relationships while maintaining clear boundaries. You should be comfortable managing a varied caseload and working within a multi-disciplinary environment.

You’ll also bring:

  • Experience (or a strong interest) in substance misuse, mental health or wellbeing services
  • Strong communication and report writing skills
  • Good organisational skills and attention to detail
  • Confidence using IT and case management systems
  • A proactive and solution-focused approach
  • Understanding of safeguarding, professional boundaries and confidentiality
