Job Title FINDING HELP FORUM COORDINATOR JOB DESCRIPTION Hours 15 hours per week – Fixed Term Contract Ends 31st August 2026 Location Cambridge City Reports to Wellbeing Manager Role Summary: The ‘Finding Help Forum’ is a multi-agency, fortnightly meeting held in person in Cambridge City, dedicated to providing coordinated support for individuals and families with complex needs. By fostering collaboration among various agencies and organisations, the Forum operates in a person- centred way. Encompassing a wide range of issues, including mental health, housing, social care, isolation, lifestyle changes, and domestic violence, ensuring that participants receive comprehensive and integrated support tailored to their unique circumstances. The coordinator of the Forum plays a pivotal role in managing these meetings. This role is responsible for organising and facilitating the sessions, ensuring effective communication and collaboration amongst partners, and supporting the overall functioning of the meeting. This role is crucial in achieving effective and efficient outcomes for participants. Enhancing health, wellbeing, and social outcomes for participants by promoting collaboration and communication among partner organisations. Principal Tasks: Working with an understanding of elements of wellbeing and other support within Care Network and in the community or on-line, and using a person-centred and strengths-based approach, you will facilitate the connecting of individuals with the support they need to improve their lives. Provide the safe space for a citywide triaging function for complex cases to reduce duplication and ensure timely access to appropriate services across Cambridge. Raise community awareness about the forum and its services. Under the guidance of the Wellbeing Manager: • Remind partners of the fortnightly in-person multi-agency meeting through engaging email communications and prepare for each meeting. • Ensure co-chairs are in place for each meeting and brief them on the agenda. Provide necessary support to co-chairs to ensure the meetings run smoothly. • Make sure each partner has the correct information, including the welcome pack, ahead of the meeting, especially if are new to the meeting. March 2026 • Support partners in the sharing of their resources with the other partners attending the fortnightly meetings through email communications. • Ensure all partners have had a ‘what matters to you’ conversation and have consent from the participant to share their details. • Maintain up-to-date contact details of invited partners and ensure consent is obtained for group email communications, storing these securely and in-line with GDPR. • Serve as the main point of contact for partners regarding the meeting and any pre-meeting questions. • Regularly review the content and develop it as a resource to help organisations understand the role of the forum. • Facilitate a meeting culture that encourages partners to build relationships and share details so collaborative working can be achieved outside of the forum too. • Note case discussions during each fortnightly meeting including the ideas suggested, the proposed next steps, any personal budget requests and the outcome of such requests. Store this information anonymously and in line with GDPR for data reporting, including on Care Network's CRM system. Support The Finance Manager with these requests where needed. • Document cases, discussions, ideas suggested, and actions with designated owners. • Gather regular feedback both formally and informally. • Conduct data analysis to generate insights and present regular updates during project meetings. • Organise training sessions as and when appropriate in partnership with others. • Follow the guidance of line manager as required, attend training, supervision and team meetings. • Actively promote the Wellbeing Service and the wider Care Network services, with relevant partners and organisations and through events, networking and raise awareness of all services, among potential clients, volunteers, referrers, and within the wider community. • Work closely with internal Care Network colleagues to maintain useful local information, such as contact numbers of other services or agencies to whom clients may wish to be referred onward to. • Keep abreast of any local issues, developments or opportunities that could affect Care Network’s services and inform colleagues at appropriate levels within the organisation. • Be aware of and comply with Care Network’s Data Security and Protection Policy, ensuring confidentiality of client information. Demonstrate a commitment to Care Network’s values and behaviours as well as Equal Opportunities, Health & Safety, Safeguarding and other relevant policies in all areas of responsibility. • Contribute case studies, reports and articles for Care Network’s contract reporting, meetings, newsletters, website and social media.
