An exciting opportunity has arisen in the organisation for the following position:

End-of-Life Community Support Programme Coordinator

This is a full-time post for 37½ hours per week

Attracting a salary of £34,434 plus £600 Essential Travel Allowance per annum

The post is initially funded for 3-years

Leading the delivery, coordination and continuous development of a VCSE end of life community support service. The post holder will be responsible for overseeing the day-to-day operation of the service, coordinating a multi-organisational workforce of community support staff, and acting as the primary system-facing lead with health, social care and voluntary sector partners.

The role is central to achieving the project’s ambition to reimagine how people are supported at the end of life, ensuring care is compassionate, personalised, equitable and well-coordinated across Cornwall and the Isles of Scilly, in line with national guidance and local system priorities.

Working from our Truro office and remotely, the successful applicant should have excellent leadership, organisational and communication skills and be experienced in working with individuals, organisations and communities. An understanding of community and/or palliative and end of life services is an advantage, and you must be able to drive and have a vehicle for use during working hours as travel throughout Cornwall will be required.

You will be able to demonstrate experience in working across multi-agency partnerships, including statutory and voluntary sector organisations, and have knowledge of mobilising and managing services or programmes across multiple partners. Confidence in building relationships across health, social care and community systems is essential.
