Our mission is to work alongside individuals in Blackpooland the Fylde Coast who feel they have no voice and are notlistened to, so they are empowered to bring about real and positivechange to their own lives, and the lives of others. The successful candidate will be responsible for proactivelyengaging with local communities, particularly those who arestruggling to understand their health and wellbeing needs,experiences and priorities, working collaboratively with partners toconnect individuals to the appropriate support. This will informongoing development and improvement of person-centred, holisticsupport across our local communities.Role Purpose:Conduct proactive face-to-face outreach within relevantcommunity settings, including door-to-door engagement, toidentify and support individuals who may benefit from support.Work collaboratively within a multidisciplinary team (MDT) todeliver person-centred, holistic solutions that integrate bothclinical and non-clinical interventions.Support the development, training and deployment ofCommunity Champions to strengthen local engagement andpromote health and wellbeing initiatives.Provide practical support to individuals by guiding,accompanying and linking them into mainstream health andsupport services to ensure timely access and optimise ongoingcare.To work in partnership with relevant organisations, businessesand stakeholders to identify and deliver additional support thatmeets the needs of communities across the locality, ensuringalignment with existing services and offers.Main Responsibilities:·Proactively identify and engage CORE20PLUS5 cohorts within thelocality, developing a clear understanding of their specificneeds, priorities,and preferences to inform targeted, culturallyappropriate and responsive support interventions.·To champion innovative, person‑centred approaches that offersupport to individuals earlier and within a familiar environment.·To maintain a strong understanding of local communitycontexts, risks and unmet needs.·To appropriately, and with sensitivity, obtain views onexperiences of using and accessing health and social careservices, as well as the broader social and economic conditionsthat shape health outcomes, utilising creative methods such asdoor knocking, community events, community groups and pop-ups. ·To capture the findings of people engaged with, using bothpaper based and IT systems.·Create and present reports on findings from engagementactivities.·To actively engage and create relationships with groups whereexperience is never heard.·Input user experience data into the organisation’s database.·Develop a working knowledge of the social care and healthneeds of individuals and communities in Blackpool and theFylde Coast.·Keep up to date with national, regional and local informationthat relates to health, social care and the wider determinants ofhealth.·Contribute to team meetings.·Deal with highly confidential/contentious information in adiscrete and appropriate manner.·To undertake any other reasonable task as requiredcommensurate with the grading of the post.Please note: Empowerment exists to respond to the needs of individuals, somany of the tasks and responsibilities are unpredictable and varied.
