Location Chesterfield and surrounding areas Employer The Elm Foundation Reports To Community Team Service Manager Hours Part Time 21 hours per week. The Adult Community Outreach Worker may be expected to work some Saturdays and evenings, as required to meet the needs of the service. Contract Permanent Contract Salary Grade 7 (£25,433 – £27,545) per annum plus 3% employer pension contribution (pro rata) Annual Leave 25 days + Public holidays increasing to 30 days annual leave after two years in employment (pro rata) Start date Immediate

Job Purpose

The Community Outreach Worker will provide specialist support to men, women and young people experiencing or who have experienced domestic abuse across Chesterfield, North East Derbyshire, Bolsover and Amber Valley.

The role focuses on risk assessment, safety planning, advocacy and emotional support, ensuring individuals and families living in the community receive appropriate interventions to increase safety and wellbeing.

The postholder will work collaboratively with statutory and voluntary sector partners to support survivors and manage risk.

Key Responsibilities

Service User Support

  • Provide one-to-one face-to-face and telephone support to individuals affected by domestic abuse.
  • Deliver emotional support, practical advice, advocacy and safety planning.
  • Manage an allocated caseload of service users within the community outreach service.
  • Provide drop-in sessions, including occasional out-of-hours sessions where required by the Service Manager.
  • Ensure legal advice surgeries and Citizens Advice Bureau (CAB) sessions are facilitated at the advice centre.
  • Respond to telephone, email and voicemail enquiries in line with service standards and protocols.
  • Manage crisis situations appropriately and respond to urgent safeguarding concerns.

Risk Assessment and Safety Planning

  • Complete risk assessments, safety plans and support plans with all service users.
  • Conduct ongoing needs and risk assessments using recognised domestic abuse assessment tools.
  • Support families using the Team Around the Family (TAF) approach where appropriate.
  • Complete Early Help Assessments where required for families and children.
  • Ensure safeguarding concerns are recorded and escalated in accordance with organisational procedures.

Referrals and Accommodation Pathways

  • Assess and triage community outreach referrals in line with organisational procedures and SafeLives standards.
  • Complete refuge referral agency checks and support the triage of refuge referrals to appropriate services.
  • Implement Derbyshire referral pathways for domestic abuse risk assessment, safety planning and specialist referrals.

Multi-Agency Working

  • Work collaboratively with statutory and voluntary sector agencies to support service users.
  • Attend core groups, case conferences and multi-agency meetings where required.
  • Represent cases referred by The Elm Foundation at Multi-Agency Risk Assessment Conferences (MARAC).

Group Work and Community Engagement

  • Deliver group programmes aimed at recovery and empowerment, including:
    • The Freedom Programme
    • Power to Change Programme
    • Voices
  • Support the delivery of workshops, presentations and awareness sessions about domestic abuse and available support services.
  • Attend meetings, conferences and networking events as requested by the Service Manager.

Case Recording and Monitoring

  • Maintain accurate and up-to-date case records in line with organisational requirements.
  • Record risk assessments, support plans and safeguarding concerns appropriately.
  • Assist with the collection of monitoring data and service reports when required.

Generic Duties and Responsibilities

  • Ensure service users receive appropriate emotional, practical and financial support in line with individual support and safety plans.
  • Work collaboratively as part of The Elm Foundation team, sharing relevant information appropriately and supporting colleagues to deliver a high-quality service.
  • Provide support, guidance and assistance to colleagues, students and volunteers, contributing to a positive team environment and helping to promote good practice across the service.
  • Assist with the induction and informal support of new staff, students and volunteers, where required by the Service Manager.
  • Work within the service performance management framework, ensuring targets and contractual KPIs are met.
  • Promote equality, diversity and anti-discriminatory practice in all aspects of work.
  • Participate in community awareness activities relating to domestic abuse.
  • Maintain accurate, confidential and up-to-date case records and monitoring systems in line with organisational procedures and data protection requirements.
  • Provide emotional and practical support to service users, ensuring that responses are professional, compassionate and person-centred.
  • Promote service user involvement and feedback to support continuous improvement and service development.
  • Maintain up-to-date knowledge of domestic abuse, safeguarding legislation, housing options, welfare benefits and relevant support services.
  • Assist with service monitoring and reporting requirements for internal and external funders.
  • Work flexibly to meet the needs of the service, including evenings, Saturdays and unsocial hours where required.
  • Ensure compliance with The Elm Foundation’s financial regulations, policies, procedures and codes of conduct.
  • Maintain awareness of security procedures, including alarm systems and CCTV where applicable.
  • Participate in supervision, appraisal, training and professional development opportunities as required.
  • Undertake administrative duties necessary to support effective service delivery.
  • Undertake any other duties reasonably requested by management.

The postholder must maintain strict confidentiality regarding service users and refuge locations, in accordance with safeguarding policies and MARAC information-sharing protocols.
