Exciting and rare opportunity for an experienced Nurse or Paramedic, to join our specialist team within St Luke's to help deliver our Strategy “Every Death Matters” with a key focus on Coordination of care for patients in their last year of life.

St Luke's Coordination Hub is an exciting new service, with the aim of supporting patients who are recognised to be in their last year of life. The aim of the coordination hub is to be a “One Stop Shop” for patients and those that are close to them. We are seeking an enthusiastic and highly motivated individual to join the Coordination Hub to provide assessment, advice, and support to patients in their last 12 months of life using telephone support and video consultation approach to coordination of care.

This is a new service and as such we need additional staff to offer great support to the new innovative in our work. We are starting on our exciting journey and our post will offer exciting new opportunities for anyone looking at working at band 6 level who are interested in ways of working who are keen to support the development of the new service and delivering high quality safe care to patients.
