Children’s Community Palliative Care Support Worker
We’re looking for a passionate Children’s Palliative Care Support Worker to join our dedicated community team and provide high quality individual support to a child and their family both at their home and in the hospice. You will provide comfort and care to support our younger patients and make a difference when it matters the most.
Key Responsibilities:
Provide patient centred personal and clinical care and support of children and young people with complex needs in accordance with the care plan in a range of settings
Deliver a high standard of specialist palliative care in partnership with other professionals
Provide an empathetic and supportive presence for families and refer to appropriate services as needed
To act as an advocate for children and their families
Engage the patient with a range of activities that meets their needs
What we’re looking for:
Previous work or personal experience in caring for children with complex health needs
Excellent verbal and written communication skills
Strong understanding of safeguarding children
Ability to work unsupervised with shifting priorities and organise own workload
NVQ Trained to level 2
Full UK driving licence
