JOB DESCRIPTION TITLE: Carers Wellbeing Officer SALARY: £24,920 per annum pro rata CONTRACT TYPE: Fixed Term until 30th September 2027 PENSION: 3% DBS: HOURS: Enhanced DBS required with Adult Barring Check 21 hours per week LOCATION: Haverfordwest EMPLOYED BY: Carers Trust Crossroads West Wales RESPONSIBLE TO: Pembrokeshire Carers Support Service Co-ordinator OVERALL AIMS OF THE POST: Working in partnership with other statutory, voluntary, and private sector organisations to identify carers throughout Pembrokeshire Provide information and support to carers enabling them to better manage the impact of their caring role and enhance their wellbeing. Work with partners to ensure carers are supported to access other existing services. AREAS OF RESPONSIBILITY To identify new carers within Pembrokeshire and actively support new and existing carers to take up services provided by statutory and voluntary organisations e.g., Pembrokeshire County Council, welfare benefits advice, support groups, etc. To provide a range of information, support, and assistance & outreach across Pembrokeshire to enable carers to better manage the impact of caring on their own health, financial and social situation. *NB this will include telephone support, video support and one-to-one support within the office base, occasional home visits and/or meeting carers in community venues. To facilitate a ‘What Matters to Me’ conversation with the carer to assess the carer’s needs for support, what the support needs are and who can deliver them. Support carers to identify and work towards outcomes they would like to achieve. Adopt a person-centred, strengths-based approach with the carer and co- produce an action plan to address the areas of identified need. When other services are more appropriate to meet the needs of carers, signpost and refer carers on to other agencies, organisations, and services, including social care for Community Care & Carers Needs Assessments. To support carers to think innovatively about short breaks by ensuring that they are provided with information on taking a break, including opportunities to take time for themselves. This information on health & wellbeing opportunities in the county and the discounts and concessions available. includes To support carers to research and access local, regional, and national grants to which they may be eligible and may support them to continue in their caring role e.g., Carers Support Fund and Short Breaks Grants. To support carers to consider and document contingency care plans, should the carer be unable to provide carer. This will include considering wider family and social support networks and support to register with the Carers Emergency Back-Up Scheme. To support the application process and increase the take up of the Carers Recognition Card by promoting the benefits to carers. To work with partner agencies and others who may have contact with carers in their caring role, to increase the number of carers known to the service and to other services in Pembrokeshire. This may include activities such as attendance at events, manning stalls, providing briefings for professionals, attending drop- ins, manning points of contact in surgeries, hospitals, etc. To work with team colleagues to identify and map out existing carer resources across Pembrokeshire and highlighting any gaps in service provision to support the development of carers support services. To raise carer awareness through involvement in carer and related community events and to be active in the planning and organisation of carer related campaigns e.g., Carers Week and Carers Rights Day. Carers Newsletter 2 To work alongside the Investors in Carers Team to promote the IiC Scheme to other organisations/settings’ and to ensure communication pathways exist with other professionals who are likely to have contact with carers e.g., GPs, health, and social care staff. to work with colleagues To represent Carers Support Pembrokeshire and attend the Pembrokeshire Carers Forum as required. Team working/Networking and Liaison To be aware of opportunities for new partnership working, and help to develop clear protocols for joint working. To attend regularly and contribute to team meetings, reflective practice sessions and action learning sets and share information and good practice with colleagues To foster and maintain existing partnerships with a wide range of statutory, voluntary and community group and forums across the county. To offer and provide team cover and continuity of services to carers in the event of colleagues planned/unplanned absence i.e., annual leave etc. Share learning and insights from your practice with the team to enable consistency and good practice Administrative/Service Development To input and maintain accurate and timely case records on our Customer Relationship Management data base To demonstrate and record outcomes achieved for carers To assist in collating carer feedback and information to evidence need for improvement of existing carers services and/or development of new services To produce case studies and carer stories to demonstrate the impact of support. To develop effective teamwork through informal and formal communication with colleagues and regular attendance and input into team meetings etc. To assist in the preparation of information reports as required by Pembrokeshire County Council, Hywel Dda University Health Board and external partners. To carry out all duties in accordance with the requirements of the Health and Safety at Work Act, adopting safe working practices always. To undertake any other duties that may be considered commensurate with the level of the post. 4 PERSON SPECIFICATION Essential Experience Desirable Direct experience of working for a support service and achieving positive outcomes for beneficiaries Experience of networking and liaising with a range of statutory and voluntary organisations Experience of supporting unpaid carers NVQ/QCF in a relevant subject e.g., Health & Social Care Understanding of relevant legislation Ability to communicate in Welsh. Experience of Information and advice giving Working with and implementing policies, procedures and guidance. Solution focussed / problem solving ability. Qualifications A good standard of general education or ability to demonstrate relevant experience. Level 3 Diploma in Advice & Guidance (or a commitment to achieve this) Skills and abilities Excellent communication and interpersonal skills An empathetic, compassionate and supportive manner and an ability to deal with queries from individuals who are distressed in a sensitive manner. An ability to communicate complex information using clear language on carers rights over the telephone, in writing or by email Highly organised working methods with an ability to write and update accurate records Ability to use IT effectively including 5 email, Word, and experience of using data bases. Ability to maintain confidentiality. Ability to use your initiative and work independently and as part of a team. Solutions focussed and problem-solving abilities Ability to plan, prioritise and manage workload. Good self-awareness and commitment to continuous professional development. Knowledge An understanding of carers and the impact of caring. A good knowledge of health and social care provisions and pathways into services Clear understanding of the Health & Safety at Work Act and own and others’ responsibilities. Other Valid Driving Licence, access to a vehicle for work purposes and ability to travel across Pembrokeshire.
