Job overview

St Anne's are recruiting a full time Care Coordinator to work at our largest service, Redwood Glades.

The Care Coordinator will have operational oversight and day to day management of the care planning system used at Redwood Glades. They will ensure all care calls are assigned to our service users in line, and maintain continuity of care to our clients.

Role is normally Monday to Friday although flexibility will be needed to drive any client care hours forward and ability to implement changes to hours.

Main duties of the job

  • To be able to provide forward planning to produce at least a 4 week rota in advance, with a solution approach to new care hours & change due to the clients ever changing needs.
  • Plan and allocate care workers to service user calls, manage their workload in line with business demands and the care worker’s availability and ensure adherence to St Annes Community Services policies, and legislation relating to Working Time Regulations.
  • Take new service user referrals from social workers and private clients and ensure they are deal with appropriately, and where required attend assessments and liaise brokerage relating to package increases.
  • Be responsible for the smooth transition of new packages of care. Ensuring all relevant details are uploaded to the system and calls are allocated.
  • Undertake care calls where required, in the event of staff absence, staff shortages or increases in demand, which may include a requirement outside of office hours.
  • Manage care worker holiday requests and ensure the correct procedure is followed and relevant documentation is completed. Responding to emergencies, care planning and activity schedules.

Experience using rota system such as People Planner is desirable.

Detailed job description and main responsibilities

This role reports directly to the First Line Manager of the service, with expected open communication with the Extra Care Senior Manager, Deputy Managers, and Team Leaders.
