About the Ascension Trust (AT) Beacon Project

The AT Beacon Project (ATBP) is an initiative of Ascension Trust, a charity registered in England and Wales, that seeks to engage communities and vulnerable groups to lead healthier lives. ATBP is an innovative programme that makes every contact count, using interactions as opportunities to improve health. The project aligns with the NHS Core20PLUS5 approach, focusing on reducing health inequalities by targeting the most disadvantaged communities.

The project works alongside individuals, their families, and community leaders in the places they live, frequent, and spend time, establishing health and wellbeing outreach interventions. Examples include activities in barbershops, social supermarkets, churches, markets, and urban housing estates.

Our clinically led neighbourhood programmes are delivered by a multidisciplinary team including nurses, mental health professionals, healthcare assistants, Beacon health ambassadors, and volunteers. Services can be accessed without an appointment or via referrals from Beacon Neighbourhood Ambassadors, other VCSE organisations, and primary care providers. Additionally, ATBP runs targeted projects such as a fruit and vegetable prescription scheme and provides focused support for individuals with identified needs, including CVD risk, through our distribution network and wellbeing workshops.

We collaborate with public health agencies, faith groups, community organisations, and local businesses within Lambeth, operating in accessible community spaces.

Core AT Beacon Health and Wellbeing Delivery

The AT Beacon nurses/allied health practitioners deliver weekly, place-based sessions to:

• Identify and raise awareness of MLTCs, frailty, and CVD risk, with guided signposting.

• Support access to services, including digital tools and the NHS App.

• Provide light-touch wellbeing conversations and practical guidance

The team also participates in community events and neighbourhood health activities to strengthen local networks, connect residents to services, and support integrated care pathways.

The AT Beacon Nurse/Allied Health Practitioner will engage in the service delivery of AT Beacon’s outreach programmes across Lambeth, focusing on communities affected by MLTCs, frailty, CVD risk, and health inequalities. Working closely with AT Beacon Project leadership, the Nurse/Allied Health Practitioner will engage service users to understand their holistic health and wellbeing needs, understand their CVD risk, and support them through appropriate signposting to Primary Care, NHS services and relevant organisations.

Using AT Beacon’s strong local intelligence and networks, the role will focus on priority health issues such as high blood pressure, diet-related health inequalities, socioeconomic disadvantage, Core20PLUS5 populations, cancer, mental health, and lifestyle-related conditions.

Key Responsibilities and Duties

Engage in AT Beacon outreach programmes across Lambeth, supporting and developing health promotion initiatives at Beacon sites.

Deliver holistic health education and screenings covering MLTCs, CVD risk, vaccine confidence, mental health, and lifestyle-related conditions.

Identify and address health concerns (e.g., raised blood pressure), with appropriate signposting to Primary Care and NHS services.

Use primary care, beacon neighbourhood Ambassadors and community networks to identify vulnerable individuals who may benefit from AT Beacon support.

Accurately document and report intervention outcomes, including measurements, referrals, follow-ups, and both quantitative and qualitative data.

Provide a culturally competent, empathetic service to individuals who may be anxious or hesitant to engage with healthcare.

Maintain patient confidentiality and adhere strictly to data protection standards.

Training and Support

The AT Beacon Nurse/Allied Health Practitioner will be supported by AT Beacon Clinical Leads, the Project Manager, and the leadership team. Support includes access to e-learning, face-to-face training, and regular check-ins with line managers to ensure professional development and effective service delivery.
