Job purpose

Our Women’s Recover Service provides safe accommodation and tailored support for women experiencing homelessness and multiple disadvantage, including the impact of domestic abuse, violence against women and girls, exploitation, substance use, poor mental health, physical illhealth and exclusion from essential services.

The Assertive Outreach and Transitions Link Worker exists to close the gap between rough sleeping, outreach support and sustained accommodation. For many women, entering a hostel is not a simple step indoors; it can mean rebuilding trust, managing risk, navigating trauma, and deciding whether services feel safe enough to stay connected to. This role makes that transition more consistent, more relational and more possible.

Working across the street, the community and the Women’s Recovery Service, the postholder will build early relationships with women before they enter the service, support informed and traumaaware assessments, and help women move into accommodation with dignity, choice and the right support around them. The role also provides assertive, ongoing support where women are finding it difficult to use their bedspace, remain connected to support, or move away from rough sleeping.

This post strengthens the service’s ability to respond to women experiencing high levels of risk, harm and exclusion without reducing them to those experiences. Through skilled relationshipbuilding, partnership working and clear risk management, the postholder will help prevent avoidable evictions, reduce repeat rough sleeping, improve engagement with specialist support, and strengthen safety for women and the wider community.

At its heart, this role is about standing beside women at the point where systems often lose them. It brings persistence, care and practical action together, so women are not just offered accommodation, but supported to stay, recover and shape the next stage of their lives.

Key accountabilities

Outreach, engagement and transitions

Build trusting relationships with women who may be referred to the Women’s Recovery Service, working across the street, community settings and partner services.

Provide trauma-informed, women-centred outreach that supports women to explore accommodation and support at a pace that feels safe and possible.

Complete flexible referral assessments in partnership with women, colleagues and external agencies.

Support women through the move into accommodation, helping reduce barriers to engagement and strengthening the chance of a sustained placement.

Provide assertive follow-up support to residents who are intermittently rough sleeping, not regularly using their bedspace, or finding it difficult to stay connected to support.

Support planning and advocacy

Work alongside women to co-produce support plans that reflect their goals, strengths, risks, choices and changing needs.

Hold a small caseload of women who need additional, flexible or community-based support to maintain accommodation and move forward safely.

Advocate with and for women to access health, mental health, substance use, domestic abuse, VAWG, social care and other specialist services.

Support women to build practical skills around tenancy responsibilities, bills, daily living and maintaining accommodation.

Risk, safeguarding and community safety

Complete clear and timely risk assessments and risk management plans, informed by direct work, street-based contact and partnership input.

Identify and escalate safeguarding concerns promptly, in line with SHP procedures and statutory responsibilities.

Respond to risks linked to serious harm, exploitation, eviction, repeat rough sleeping, anti-social behaviour or community protection concerns.

Work with colleagues and partners to manage risk in a way that promotes safety while maintaining women’s dignity, choice and voice.

Partnership working

Build strong working relationships across the Camden Homeless Pathway, including outreach teams, women’s services, domestic abuse services, adult social care, health services, police and community safety partners.

Attend and contribute to multi-agency meetings, case reviews, community safety forums and ward panels where relevant.

Share learning from direct work with women to strengthen pathways, improve joint working and challenge barriers to support.

Champion best practice in supporting women experiencing multiple disadvantage, including the impact of trauma, VAWG, exploitation, substance use, mental health needs, physical health needs and neurodivergence.

Recording and service improvement

Maintain accurate, timely and up-to-date records on SHP’s Inform system and any other relevant databases.

Record contact with women, professionals and partner agencies in line with data protection, confidentiality and case management requirements.

Contribute to monitoring, evaluation and reporting so the service can evidence impact and identify areas for improvement.

Support a culture where women’s feedback, experiences and ideas are actively heard and used to shape service delivery.

Teamwork, health and safety

Take an active role in team meetings, reflective practice, supervision and annual appraisal.

Work collaboratively with colleagues to problem-solve risk, share learning and strengthen the service’s response to women with complex and intersecting support needs.

Follow SHP and service policies around lone working, health and safety, safeguarding, professional boundaries and safe practice.

Contribute to a positive team culture rooted in inclusion, accountability, compassion and belief in women’s capacity for change.

Experience and Knowledge

Experience of supporting women who are homeless, rough sleeping, and facing multiple disadvantages such as domestic abuse, substance misuse, poor mental and physical health.

Experience of carrying out assessments, support planning and risk management.

Experience of trauma-informed, strengths-based, and psychologically informed approaches.

Knowledge of safeguarding, risk management, and the causes and impacts of homelessness.

Understanding of how statutory and community services operate, and familiarity with housing, health, and social justice laws and policies.

Experience working in partnership with housing, health, enforcement, and other agencies.

Skills and Abilities

Strong organisation, time management, and emotional resilience.

Able to analyse behaviour and risks to create effective support plans.
