Job Description
We are seeking a compassionate, organised person who will work with unpaid
Carers, Young Carers and their families to identify and access supports that will
assist them within their caring role creating their own Personal Plan or Young
Carers Statement. A key task will be to promote social inclusion by facilitating
opportunities for Carers to build networks of support so that they are more
connected within their community and have links with other people and
resources that are important to their health and well being.
Key Responsibilities
• Working with Carers to assess their needs creating a Personal Plan or Young
Carers Statement identifying outcomes that will support them in their caring
role. Reviewing regularly to ensure that support is relevant and effective.
• Delivering a range of practical and emotional supports for Carers so they will
feel able to maintain their caring responsibilities whilst also achieving positive
wellbeing
• Negotiate and liaise with services to ensure Carers needs are addressed.
• Working closely with a broad range of services including Statutory, Education
and Third sector organisations throughout Glasgow to build new and strengthen
existing relationships with partners raising the profile of GAMH North East
Carers Service and Glasgow Carers Partnership.
Identify and participate in events and networking opportunities to raise
awareness of Adult and Young Carers ensuring they are identified and offered
support early in their caring journey.

• Ensure adherence to GAMH policies and procedures including safeguarding
and Data Management protocols.
• Collect and maintain accurate and up-to-date records relating to the service
including entry onto electronic spreadsheets and databases, and support
information retrieval and report writing.
• Any other duties appropriate to the post and in accordance with the
requirements of GAMH

Our team provides flexible intensive family support across Glasgow that enables children and young people on the edges of care, and their families, to receive the right support at the right time, building on their strengths.
The Family Support Worker will work in partnership with the Health & Social Care Partnership and other family support providers to share resources, learn and create solutions that will help children, young people and families to thrive.
This role is part time working 25 hours per week, for a fixed term period of 18-months.
Main duties and responsibilities will include:
- Provide peripatetic support to our Children and Families services in Glasgow, Young People and Family Support and Common Ground
- Provide a minimum of 2 Common Ground outreach shifts per month. Shift times are Friday and Saturday 4pm to 10pm in Glasgow City Centre
- Develop good communication and working relationships with children and families, colleagues and other professionals
- Work alongside all immediate family members and extended family/support network as required
- Support children and families in the local community
- Deliver evidenced based family support that helps families overcome current challenges and build resilience for the future
- Maintain accurate case files for all family members and consistently assess progress across all outcomes
- Plan, implement and continuously develop packages of tailored interventions for families to meet the needs of the individual child, their parent(s)/carer(s), siblings and wider family network, including practical support, parenting education and support, and emotional health and wellbeing
- Assist individuals accessing the programme to engage and integrate into their local community
- Undertake whole family assessments that focus on strengths and continuous assessment of progress towards outcomes for all family members

Location

Experience of working within substance misuse and criminal justice knowledge or a related Health and Social Care setting is essential as well as experience of delivering structured treatment groups. You must be committed, resourceful and highly motivated to engage people within treatment services and promote recovery through their daily practice.
You will be co located within Chester probation offices and our Chester Hub as part of Dependency and Recovery to improve continuity of care upon release from prison to achieve people who use our services recovery journey goals, via 1:1s and structured groupwork, and case manage clients on probation alongside the Probation Practitioners.
Your duties will include:
- assessing their needs, delivering effective interventions, completing detailed reports, and working closely with our criminal justice partner agencies, clinical team and other partner agencies.
- conducting assessments for clients, including Alcohol Treatment Requirement (ATR) and Drugs Rehabilitation Requirements (DRR) assessments.
- managing a case load and coordinating and delivering assessments and evidence-based interventions (brief and structured), recovery planning and aftercare planning.
- delivering harm reduction advice, psychosocial interventions, needle & syringe exchanges, Blood borne virus testing and interventions, drug testing and overdose prevention advice.
An empathetic and non-judgmental approach is imperative, as this role involves working with a wide range of challenging issues and behaviours. Good time management and team working skills are essential in this fast-paced service.
