As an Evening and Weekend Support Practitioner, you will work on a rota-system in the evenings and weekends, including Bank Holidays, working extra shifts when required to cover sickness and annual leave. We are looking for an experienced Drug and Alcohol Practitioner to join our Chesterfield team, managing a caseload of service users presenting with drug and alcohol issues in the North East Derbyshire area of the Derbyshire county. The role will involve offering a range of harm reduction interventions and structured key working, helping engage service users in a care-planned and recovery focussed way to attain improved health outcomes and achieve their personalised treatment goals. This will involve you effectively organising and delivering 1-2-1 care-planned support and facilitating group work sessions as required, using appropriate models, interventions and approaches tailored to meet the individual needs of service users. Bradford RESET service, which stands for Recognising, Engaging and Supporting Everyone to Transform. This innovative service brings a trauma-informed approach to support individuals who have caused or who are causing harm in their intimate and/ or family relationships and their families and for victims with additional complex needs referred through the Multi-Agency Risk Assessment Conference (MARAC)
Bradford RESET uniquely combines our Navigator existing services focused on domestic abuse, delivered by Bridge, with an extensive new programme offer for individuals aged 16 and over who cause harm. This integration creates a vibrant opportunity for Bradford to tackle the underlying causes of domestic abuse by providing effective, therapeutic, and proactive support. We recognise the importance of holistic family wellness and safety, offering therapeutic support for entire families affected by the behaviours of individuals involved in the programme.
This post is for a MARAC Navigator and focusses on a selected group of victim cases that return to the MARAC due to repeated high-risk domestic abuse incidents where engagement with either the victim or perpetrator in the co-ordinated action plan has been unsuccessful. The MARAC Navigators will receive all referrals from the MARAC meetings, working with a small caseload of typically 10 cases each. MARAC Navigator will aim to:
- Provide aspirational person-centred support through building a trusting relationship with the victim
- Pathway any cohobating people who harm to a navigator within the people who harm element of the Bradford RESET service to enable them to address issues that contribute to incidents of abuse
- Proactivity and intensively conduct face to face work with the victim to build all the networks of support that wrap around the individual to facilitate recovery, building and sustaining hope
- Work as part of a wider team to build and evidence what future support should look like.
A Multi-Agency Risk Assessment Conference (MARAC) is a weekly meeting where information is shared on the highest risk domestic abuse cases between representatives of local police, health, child protection, housing practitioners, Independent Domestic Violence Advisors (IDVAs), probation and other specialists from the statutory and voluntary sectors. At the heart of a MARAC is the working assumption that no single agency or individual can see the complete picture of the life of a victim or perpetrator and any children, but all may have insights that are crucial to their safety. After sharing all relevant information, they have about a victim, any children and the perpetrator, the representatives discuss options for increasing safety and turn these into a co-ordinated action plan.
Main Duties and Responsibilities
- Assertively make face to face contact and proactively engage with referred individuals through all routes such as, meeting at an individual home, Bridge Project, friends or family members home or any agreed place.
- Provide creative person-centred trauma informed solutions to the unmet needs of individuals who are hard to reach and engage.
- Build a trusted, motivational and highly supportive relationship, individualising support through undertaking a mutually agreed personalised review of their strengths and support needs and levels of wider social and community engagement.
- Mutually identify and work together to overcome barriers and access a wide range of health and wellbeing services, providing appropriate advocacy. Working in a trauma informed way, use high support techniques to affect recognition of DA and individuals to achieve their goals
- Proactively link and practically support individuals into the agreed appropriate agencies, services and community assets relevant to their expressed needs and the level of risk e.g. physical and mental health, sexual health, housing, drug and alcohol services, benefit claims, criminal justice services, employment, training and education programmes and any specialist support services as required etc. this will include transporting people to their appointments.
- Liaise on a regular basis with the Police DA&SV team to discuss and review cases.
- Maintain appropriate case notes and administrative systems linked to the programme, including maintaining programme data performance/ evaluation systems.
- Produce high quality, accurate written reports and letters of support as and when required, this routinely includes reports for multi-agency bodies and letters outlining individual support needs as part of the overall package of care.

Leukaemia Care is the UK’s leading leukaemia charity. For over 50 years, we have been dedicated to ensuring that everyone affected receives the best possible diagnosis, information, advice, treatment and support.
We are here for everyone affected by leukaemia and related blood cancer types – such as myelodysplastic syndromes (MDS) and myeloproliferative neoplasms (MPN).
We provide information, advice and support for anyone affected, this includes patients and their friends and families too. We raise awareness of the issues impacting people affected by leukaemia, MDS and MPN, and campaign to fix them. We are driving early diagnosis, raising public awareness, improving services and ensuring access to effective treatments.
How do we do this? We have a wide-range of services aimed at patients and their loved ones from diagnosis and beyond. Our support services range from nurse-led helplines, support groups and free-of-charge medical information to access to paid counselling, grants for those feeling the financial strain of a diagnosis and providing buddies so people always have somebody to talk to who have experienced something similar to them. Crucial to our work is raising awareness of leukaemia as a blood cancer and the importance for early diagnosis. Leukaemia can be difficult to spot as the symptoms are similar to many other common conditions. Unfortunately, most people are not aware of the signs of leukaemia and this leads to diagnosis being delayed, which can worsen outcomes for patients.
As well as working to educate the general public through our #SpotLeukaemia campaign, we are working closely with our colleagues working in front line medical services to ensure they have the most up-to-date information on diagnosing leukaemia. We know that by raising awareness and working to educate health care professionals, we can save lives and improve outcomes.
Overview
This is an excellent opportunity if you wish to develop your career in patient support and services. Any necessary training will be provided for the successful candidate. You should be proactive, enthusiastic, and excited about becoming part of a team looking to develop and implement the charity’s plans for the patient service team.
You will provide patient support and raise awareness of the services we have at Leukaemia Care at the hospital. You will talk to patients, carers and family members, offering support, information and clear signposting to our services and other relevant organisations.
You will be based on the haematology ward/clinic and this is a non-clinical role. You will be required to record all your work so administration is part of this role.
A laptop and mobile phone will be provided, along with patient resources. You will be reimbursed for parking at the hospital.

DENS Day Centre provides a safe and welcoming space for individuals experiencing homelessness, poverty and social exclusion. As a Day Centre Support Assistant, you’ll be at the heart of our mission, offering practical help, emotional support, and a listening ear to those who need it most.

As a charity we provide on-going mental and emotional well-being support to families affected by childhood cancers, both on and off treatment, but particularly recognising that post treatment can be a challenging time, when families are having to adapt to their `new normal’. To date, we have supported over 350 families across Cheshire, North West England and remotely in North Wales. After 14 years of fundraising, we opened our brand-new purpose-built Joshua Tree Support Centre in May 2021. The centre, located in Sandiway, Northwich has been built and furnished entirely through a mix of local donations, grant funding and corporate support. There are now a range of exciting opportunities to further grow and develop the charity over the coming years. This includes increasing the number of families we support, diversifying and raising new income streams and further developing our partnership work. Job Purpose To deliver play based activities at the Support Centre in Cheshire and/or on location at identified venues across Cheshire. Work alongside our family support team to develop and plan playful events that enable children and families to be included in fun, child-centred activities and build friendships. To create opportunities to embed principles of quality play into events and supervise child-led play. The activities will be delivered during evenings, weekends and school holidays, therefore offering families a broad range of opportunities to engage. Days and hours of work will be negotiated at least a month in advance. As part of our family support service, The Joshua Tree provides opportunities for all members of our families to connect with and gain support from our team of support workers. This includes children and young people that are newly diagnosed with cancer, on treatment and post treatment, as well as siblings and wider young family members. Key Responsibilities • Develop a range of play based activities aimed at engaging families who are affected by childhood cancer. • To use knowledge of child development and the benefits of play to deliver and evaluate inclusive activities that will enhance children’s emotional wellbeing. • To engage and communicate effectively with families.
