At Prospect Hospice, we believe that everyone deserves outstanding, compassionate care at the end of life. Our highly skilled Single Point of Contact (SPoC) Team is the first point of contact for patients, families and healthcare professionals seeking specialist palliative care advice and support.
We’re looking for an experienced and motivated SPoC Clinical Practitioner to join this innovative team — providing expert clinical triage, holistic assessment and timely guidance to ensure that patients receive the right care, at the right time.
This is a dynamic role ideal for someone who thrives on variety, clinical responsibility, and meaningful impact.
The Role
As a SPoC Clinical Practitioner, you will provide a central, telephone-based access point for hospice care—ensuring patients and professionals receive timely, high-quality advice and support.
You will:
- Triage referrals and assess urgency to ensure appropriate and responsive care
- Provide specialist clinical advice and emotional support via the advice line
- Undertake holistic telephone assessments to identify patient needs
- Support advance and anticipatory care planning
- Work collaboratively with multidisciplinary teams across health and social care
- Maintain accurate, confidential electronic patient records
- Contribute to clinical governance, audit and service development
This is a varied and rewarding role where your clinical expertise and decision-making will have a direct impact on patient care.
Hours: 22.5 hours per week, 9:00am – 5:00pm on a rota basis across Monday to Friday
What we’re looking for;
We welcome applications from a range of experienced clinicians, including:
- Registered Nurses (NMC)
- Occupational Therapists, Paramedics or Allied Health Professionals (HCPC)
You will have:
- At least 5 years’ post-registration experience
- Experience or strong knowledge of palliative or end-of-life care
- Excellent communication and telephone triage skills
- Confidence in clinical decision-making and risk assessment
- A compassionate, patient-centred approach
Experience in community, urgent care, or complex care environments would be highly beneficial.
