Our innovative NHS programme (High Intensity Use (HIU) Programme) helps people who have physical or mental health issues and access health care more than expected. The aim of the programme is to change the perceptions of people who access health care more than expected. The team help to prevent people from reaching crisis point, empowering them to better self-manage and connect with their surroundings.

We support people who are on the edge of care, or whose needs are escalating, to maintain their independence and improve their well-being . We respond to the needs of people who may be vulnerable, in crisis or struggling to navigate services. We help them to maintain a network of external relationships across health, social care, emergency services and the voluntary community sector. 

  • Providing person-centred assistance via the telephone or face-to-face in the community.
  • Liaising with partnership agencies to advocate for people affected by a range of unmet needs, social isolation and loneliness. You will assist them accessing services in order to reconnect with their communities, maintain or regain independence and improve well-being.
  • Building connections with local voluntary sector organisations to gain knowledge of the services they provide and link local residents with them.
  • Ensuring that the service is efficient, reliable and delivered consistently with national and local guidelines
  • Using a range of IT solutions to capture case notes, identify support options, and communicate with colleagues and partners.
  • Experience of person-centred support planning, helping individuals to identify goals and achieve positive outcomes.
  • Proven track record of providing exceptional public services.
  • Confidence in using IT software.
  • Excellent time management and the ability to prioritise work and manage a proactive and reactive workload.
  • Strong interpersonal and communication skills, relationship building, active listening with a wide audience.

  • You will be expected to travel to the following boroughs as/when required: Harrow, Brent, Ealing, Hammersmith and Fulham, Hounslow, Kensington and Chelsea and Westminster. Although these may be subject to change, it's expected cover will continue to be required in boroughs in the north central and northwest areas of London (north of the river Thames)
