1.  Job Summary

You  will  enable  and  support  people  to  make  positive  decisions  about  their  lives,  promote  their
independence and ensure they can get involved in their local community.

Working in the team, you will be one of the first points of contact between the public and the service,
gathering information, identifying needs and completing assessments, as well as sign posting people
to other services.  You will work alongside the qualified practitioners with a variety of tasks, including
managing your own case load, assessing people’s needs and carrying out regular reviews under the
relevant legislation.

One of the purposes of this role is to help ensure and maintain the high-quality service provision that
the organisation offers. In your role, you will support the facilitation of transition pathways for new
admissions to the service, plus contribute and participate in meeting the needs of existing people in
service.

3.  Key Tasks

  To work jointly with the  team and using one’s own experience and initiative on applications to

Kisharon Langdon for services and support;

  To work in accordance with the Kisharon Langdon assessment processes;
  To work collaboratively with the team in delivering Kisharon Langdon’s aim to provide personalised

services for members;

  To  collect,  accurately  record,  manage  information  systems,  critically  analyse  all  relevant
information  and  take  appropriate  actions  to  fulfil  statutory  responsibilities  to  promote  and
safeguard the well-being and interests of the Kisharon Langdon members;

  To complete Care Act 2014 compliant assessments and reviews;
  To  work  with  individuals  in  accordance  with  Care  Act  2014,  statutory  guidance  and  other
appropriate legislation such as the Mental Capacity Act 2005 and the Human Rights Act 1998;

  To build partnership networks with other organisations such as the Local Authority;
  To raise awareness of our organisation and develop valuable links for the team and service.

4.  Operational Tasks

  To ensure referrals and enquiries are responded to in a timely manner;
  Coordinate and attend admissions meetings;
  To support and have oversight of transition pathway, this includes development and coordination

of transition plan;

  To support the facilitation of assessments for both new and existing Kisharon Langdon members;
  To liaise and work with other partners, professionals and agencies to achieve optimal outcomes

for Kisharon Langdon members;

  To ensure that services have a coordinated and collaborative approach;
  To contribute to the Safeguarding processes;
  To contribute to Kisharon Langdon’s Deprivation of Liberty (DoLs), process as and when required;
  To work with Kisharon Langdon housing team to source suitable accommodation;
  To review finances, benefits and funding streams as applicable with any family and/or member;
  To attend placement reviews and support with the preparation of review documentation, as and

when required.

  To support team members to create and amend members risk assessments, care plans and other

relevant documentation;

  To attend professionals’ meetings, both internal and external at a variety of bases;
  To be able to commute as required within the role;
  To actively support the team by undertaking any other duties commensurate with the job or needs

of the service that may not be explicit here.

5.  Communications

  To be able to accurately record and articulate assessments and information as required;
  To  be  able  to  communicate  using  accurate  evidence-based  information  in  accordance  with

statutory and organisational requirements;

  To communicate effectively to a variety of audiences through written, verbal and other means of

communication;

  To have competent IT skills.
