Are you passionate about making a real difference in people’s lives? Do you thrive on helping others overcome challenges and build brighter futures? The Nelson Trust is looking for a dynamic, compassionate Community Connector to join our Recovery & Aftercare team!
This role involves supporting clients who have completed treatment and need help to further develop their recovery capital. You will act as a bridge between structured treatment and aftercare, ensuring clients receive compassionate and collaborative services.
- Conduct individual assessments and develop personalised recovery plans.
- Provide crisis intervention and assist clients in accessing necessary services such as mental health support, housing assistance, and employment resources.
- Facilitate participation in mutual aid groups and provide information on addiction and relapse prevention.
- Monitor progress, advocate for clients, involve family members, and support volunteer activities.
- Report all safeguarding concerns and support the delivery of recovery coaching and peer mentoring programmes.
- Coordinate recreational activities and community events.
- Experience in substance misuse support and needs assessments
- Lived experience of addiction & recovery is highly valued
- Strong understanding of safeguarding and recovery capital
- NVQ Level 3 or above (or willingness to work towards it)
- Excellent communication, IT, and listening skills
- Warm, approachable, and a true team player
- Ability to build rapport and maintain professional boundaries
- Commitment to confidentiality, equality, and safeguarding
- 35 hours per week
- 25 days holiday + statutory
