Deputy Clinical Lead - End of Life Urgent Care Service (EOLUCS) (Band 6)

Hours: 37.5 Hours Per Week

Salary: £37,715 - £45,416 Per Annum, Dependent on Experience

An exciting opportunity has arisen for enthusiastic, well-motivated professional to specialise in end-of-life care.

As the Band 6 Deputy Clinical Lead, you will be supporting the Clinical Lead for EOLUCS to Manage the operational management of the EOLUCS team whose aim is to support community-based end of life (EOL) patients in meeting their preferred place of care/death and prevent inappropriate admission to secondary care.

The post holder will support the clinical decision making for those patients under the care of EOLUCS, undertaking specialist holistic assessment of patient/family needs in partnership with the patient and their family/carer as required. Developing and being responsible for networking and building relationships with both internal and external health and social services to ensure patients receive appropriate joined up care as part of a system wide approach to EOL care within the catchment area we serve as a Hospice.
