NHS North East North Cumbria Integrated Care Board (ICB) is a key part of the NHS in our region, working to improve the health of our local population. We are seeking experienced qualified nurses to join our All Age Continuing Care Service based in Carlisle.

We have exciting opportunities available within the ICB for Band 6 Nurse Assessor/Case Officers, in the North Cumbria All Age Continuing Care team. The team is part of the North East and North Cumbria ICB North region covering the areas Northumberland, North Tyneside, Newcastle Gateshead and North Cumbria. The posts are open to nurses with RGN, RNLD or RMN registrations. Reporting to the All Age Continuing Care Clinical Lead Nurses, you will be part of a dedicated team working within the health and social care system.

Main duties of the job

Committed to high quality patient and family experiences, you will be working collaboratively alongside other partners and stakeholders to ensure individuals needs are appropriately met. You will be responsible for the specialist Continuing Healthcare assessments, reviews, and case management of the health needs of individuals either in their own homes, residential, specialist accommodation or nursing accommodation and act as a point of contact, referral, and advice, completing regular reviews for health funded individuals and ensuring compliance with NHS National Framework Guidance. Case Management may also include safe commissioning of safe services for individuals, supporting with Court of Protection & Deprivation of Liberty processes, engagement in safeguarding, appeals, professional disputes and complaints, support to find appropriate placements etc.

Detailed job description and main responsibilities

You will be expected to have excellent communication skills, be able to work with families, carers, and other stakeholders in a complex and challenging but supportive work environment. Whilst experience of involvement in the All Age Continuing Care processes is desirable, this is an excellent opportunity for nurses to increase their knowledge and understanding and enhance their skills whilst being part of an experienced and supportive team.

Person specification

Knowledge, Training and Experience

Essential criteria
  • Registered Nurse on NMC register
  • Evidence of degree level study or equivalent
  • Evidence of working within a Multi Disciplinary Team
  • Evidence of assessing individual patient needs, using recognised National CHC assessment tools
Desirable criteria
  • Experience of working within CHC arena and specifically Appeals and retrospective service
  • Community nursing experience
  • Experience of Independent Sector Nursing
  • Experience of delivering training
  • Training qualification/ENS 998 or equivalent
