Job overview
The Discharge Nurse will be responsible for facilitating safe and
effective discharges of complex patients from hospital to a community setting through a multidisciplinary approach. They will co-ordinate assessments of patients and support the discharge plan.
The Discharge Nurse will have a caseload of the most complex
clients and will be responsible for completing and coordinating assessments and discharge plans for this group of patients, ensuring that all parties are updated at all times.
This group of patients Includes but is not exclusive to
Homeless patients,ensuring that the correct pathway is followed and that the trust are in receipt of any changes to legislation or the process
Complex patients requiring either specialist rehabilitation or
placement ensuring that the correct pathway and funding streams are applied for and adhered to incorporating any changes to legislation or process.
Overseas patients with no access to public funds requiring intervention from the Home Office or repatriation to their country of origin
The postholder will be responsible for providing specialist advice and support to the wards and the MDT’s in managing effective discharge. They will take the lead in Family and Professionals meetings providing guidance and support to both professional colleagues and patients/ families
Main duties of the job
Active input and support to the MDTs for each ward and ensure feedback on actions assigned.
To facilitate effective multi-disciplinary discharge planning and processes as an integral part of care management throughout all the divisions.
Demonstrate a high level of clinical judgement and act autonomously Tracking and assistance in completion of referrals where necessary, of all
patients referred to ensure supporting referrals are completed in a timely and efficient way and that they progress on to the next stage of their pathway as soon as possible.
Ensure that patients that are going to another provider for care, do so with the right information, consumables and equipment.

standard of patient care.

To be accountable for own professional actions and act as a role model.
To act as a resource and educate and inform others in area.
To attend any such training courses that are required, to extend knowledge and skills and to keep up to date with professional developments.
To be responsible for the dissemination of information regarding new or
updated clinical practices, guidelines and policies to all nursing staff.
To participate in training needs analysis for own area.

Education & professional Qualifications
Essential criteria
- Registered Healthcare Professional
- Evidence of Continous Professional Development
- Healthcare Related Diploma/Degree in working towards or relevant portfolio of experience
Experience
Essential criteria
- Demonstrate an understanding of the needs of adults with complex physical and/or cognitive ability and their carers
- Knowledge of relevant national guidelines and legislation around Discharge Planning
- Understanding of the revised frame work in NHS Continuing Health Care/NHS Funded Care

- Ability to work under pressure and achieve tight deadlines in a complex/changing environment
