Dementia Wellbeing Connector Pembrokeshire

Salary: DP4 pay - £25,298 FTE.

Hours: 21 - 35 hours per week.

Place of Work: Hybrid working model from home or the Haverfordwest
office, with travel throughout the county to client homes.

Contract:  Currently  through  until  March  31st  2026  potential  for
extension if funding allows.

We are looking to recruit a self-motivated, enthusiastic, hard-working team
member to join our exciting new service that supports people living with
Dementia (PLWD) and their carers. The service aims to provide full wrap-
around support that will assist the PLWD in navigating their individual journey
and increase their overall wellbeing as much as possible.
