This is an exciting and innovative time as a Transfer of Care Hub continues to develop to meet the recommendations of the Department of Health's Hospital Discharge Service: Policy and Operating Model. This guidance encourages a Home First and Discharge to Assess culture and reinforces the embedding of a Single Point of Access and Home First ethos as best practice.
We are looking for nurses, therapists, social workers or pharmacists that would like to work in the Transfer of Care Hub to develop an integrated approach to hospital discharge and pathways to access community services across the local health and social care system.
Main duties of the job
As a Discharge Facilitator you will provide expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patients, resulting in the transfer of the patient to the most appropriate level of care for their needs.
The Discharge Facilitators will also work with other members of a multi-disciplinary team in order to provide a patient-centred service working to identify patient and carers needs to facilitate a safe and timely discharge.

Knowledge & Skills
Essential criteria
- • Excellent written and verbal communication skills with the ability to converse in a positive manner with all levels of professional colleagues, carers and patients
- • Negotiation and facilitation skills
- • Excellent Knowledge of Health and Social Care services, current issues and trends relating to discharge.
- • Registered Nurse/Occupational Therapist/Physiotherapist with specialist training or experience of discharge management in an acute hospital setting
- • Aware of current developments in discharge agenda
- • Previous involvement in preparing reports relating to discharge delays.
- • Excellent understanding of the importance of discharge planning, the discharge process and potential obstacles.
- • Computer literate with good computing skills and ability to use programmes such as Word and Excel competently.
- • Display sound analytical and critical thinking ability in complex and rapidly changing situations and develop a range of options
Qualifications & Training
Essential criteria
- • Nursing/Occupational Therapy/Physiotherapy/Social Work Degree or evidence of working towards
- • Specialist training or experience in discharge management
- • Registered Nurse/Occupational Therapist/Physiotherapist with specialist training or experience of discharge management in an acute hospital setting
Experience
Essential criteria
- • Experience of working in Nurse led discharge
- • Previous experience with discharge planning
- • Previous involvement in preparing reports for discharge delays
- • Experience of resolving complex discharges across more than one area where trust wide issues arise.
- • Experience with End of Life care management

You must have appropriate UK professional registration.
