The Project:

This project is funded for a fixed term to support young people from the SWALLOW Youth Group (or other young people) to learn fundamental life skills during the school holidays, helping to bridge the gap between our Youth Group and Base House attendance.

Job Purpose:

· The Base House Transition Project Coordinator will lead and deliver a practical life-skills program for young people aged 16+ with learning disabilities. The project is designed to support young people—primarily those transitioning from our Youth Group—by helping them develop the skills, confidence, and independence needed for adulthood.

· The role will focus on creating a safe, supportive and engaging learning environment within a family-style training home setting. Through practical, hands-on activities such as cooking, shopping, budgeting and general daily living tasks. The coordinator will guide young people as they gain the essential skills required for more independent living.

· The aim of the project is to act as a bridge between the SWALLOW Youth Group and progression into Base House overnight stays, where individuals learn to manage everyday responsibilities while living alongside peers.

Key Responsibilities:

Working closely with our youth club, consulting with the young people that attend you will be responsible for planning, co-ordinating and delivering the Base House Transition Project programme. The project will run 4 days a week (Monday-Thursday) 10.00-14.00 and the focus will be on learning new life skills while becoming familiar with the Base House setting.

Main Duties:

  • Helping to aid smooth transition from children’s services into adult services.

  • Facilitate practical, real-life learning experiences. Sessions will also cover important topics including budgeting, health and safety, personal hygiene, and building positive social relationships.

· Encouraging and engaging young people to take an active role in shaping the program- ensuring that the project is user led and adapted to individual abilities to help build confidence, ownership, and enthusiasm.

· Work closely with families, schools, and other SWALLOW staff

· Help to develop information and resource pack for parents and carers, outlining local opportunities, services, and pathways available to young people after leaving school. This will help families make informed decisions and plan appropriate support packages for the future.

  • Support the young people attending to work towards personal goals linked to independence and transition to adulthood.

  • Monitor progress and outcomes against the project aims.

  • Gather feedback to make continuous improvements and prepare reports for funders.

  • To ensure all the correct paperwork is in place, including attendance sheets and progress reports for individuals.

  • To attend the youth club on a regular basis, supporting as a staff member when required to help build positive, trusting relationships with the young people that attend and their parent/carers.

  • To complete risk assessments for individuals or group activities as required.

  • Lead and guide support staff that are involved with the project.

Safeguarding:

  • Enhanced children’s DBS check will be required for the role.

  • Ensure the safety and wellbeing of young people at all times, following the SWALLOW child protection policy.

Session Details:

· Each project day will typically run from 10:00–14.00, supporting a group of three young people per session.

· Activities will include welcome and check-in sessions

· Community outings such as shopping trips, shared cooking and meals, and structured learning activities focused on life skills.

· You will have an allocated support worker to work alongside you at the sessions.

Person specification for Base House Youth Transition Project Leader

Essential qualities:

  • Experience of working with young people with learning disabilities in a training/supporting role.

· A strong commitment to inclusive, person-centered practice.

  • An understanding of transition processes and challenges faced by young people moving into adulthood.

  • Excellent planning and delivering skills.

· Confident in facilitating group activities, encouraging participation, and supporting individuals to try new experiences in a positive and safe environment.

· Organised and creative with the ability to adapt sessions to meet the needs and interests of each group.

· Strong communication skills, as the role involves building relationships with young people, families, schools, and support professionals.

  • The ability to communicate with young people with learning disabilities in an accessible manner.

  • To have a good understanding of safeguarding, equality and inclusive practice.

  • Good leadership and teamworking skills.

  • The ability to work independently and manage priorities.

  • Monitoring and reporting skills.

  • Must be willing to undertake all relevant training as part of the induction process

Desirable qualities:

  • Level 2 or above qualification in childcare or play work.

  • To have experience of being part of a user led organisation.

  • To have been involved in the application of support plans and risk assessments.

  • Ability to display conflict resolution skills.

  • Previous first aid and child protection training.
