Job overview
We are seeking a motivated and experienced Clinical Triage and Unplanned Care Nurse to join our Community Referral Hub (CRH) team on a 22.5-hour contract.
The role is primarily based within the CRH, with the expectation of working on-site to support safe and effective service delivery. Remote working may be considered only in exceptional circumstances.
The postholder will play a key role in supporting admission avoidance across Herefordshire, with a focus on Community Nursing Triage. You will take calls that require a clinical triage and response, offering timely advice using your professional judgement and clinical expertise. Where calls require an urgent visit, you will ensure referrals are appropriately allocated to locality community nurses or another clinician within the Urgent Community Response.
As the CRH is the main access point for urgent referrals from health and social care professionals across Herefordshire, you will be central to coordinating care and ensuring patients receive the right intervention at the right time.
The successful candidate will have a solid understanding of community nursing practice and the wider community services available, enabling effective decision-making and safe patient care.
Main duties of the job
Clinical Responsibilities

Provide telephone-based nursing assessments, using advanced clinical reasoning to ensure safe and timely responses.

Offer specialist advice and clinical guidance to patients, carers, and professionals.

Use skilled questioning, listening, and communication to manage triage calls effectively in a pressurised environment.

Coordinate care with community nursing teams, urgent care services, and other agencies to ensure patients receive the right care at the right time.

Maintain accurate clinical records, follow policies and evidence-based guidelines, and support delivery of a 7-day service.
Research & Audit

Contribute to clinical audit, service evaluation, and quality improvement.

Support service development by reviewing data, identifying improvements, and applying research findings.
Training & Development

Maintain own professional development and CPD in line with NMC standards.

Participate in training, supervision, and appraisal.

Support the education of students, health professionals, and care staff.
Administrative & Managerial Responsibilities

Ensure administrative processes support safe and effective service delivery.

Work collaboratively with colleagues to minimise risk, ensure timely response to referrals, and contribute to recruitment where required.
