Job Description and Person Specification JOB PROFILE POST: SALARY: HOURS: Trilogy of risk Support Worker £24,000 - £25,396 FTE depending on experience and qualifications 37 hours per week (Full time) LINE MANAGER CEO with operational oversight by the Ceredigion Council Trilogy of risk Front Door Co-ordinator RESPONSIBLE FOR: No line manager responsibility FUNDED BY: This post is funded through Ceredigion Council Funding JOB PURPOSE Porth Gofal is the integrated decision-making hub for Ceredigion that triages and determine the level of need of all cases and the best outcomes for those individuals. Porth Gofal is the heart of service provision that focuses on ensuring that Families receive the intervention that is best placed to meet their needs or where else to guide them to early help or specialist services. Porth Gofal aims. • Children to receive active care and support both for their immediate development and ongoing wellbeing. What Porth Gofal provides: • A responsive service to family’s living in Ceredigion. • Applies a Strength based needs assessment focusing on service user choice and control using the Signs of Safety framework. • Legislatively compliant service that meets the statutory requirements of the SSWBA. • A safe service that identifies vulnerable and at-risk family’s that responds effectively. • be the first and only point of contact for residents and key partners. • design and deliver early prevention and intervention packages bespoke to individuals’ needs. • decide upon the most appropriate services to deliver support in a time-related manner. • monitor and evaluate the impact of support. • Ceredigion Trilogy of risk model will be one of partnership working with DDAS and WWDAS, (specialist services) and Social Care in order to achieve the desired outcome of early help and prevention of escalation where there is concern surrounding trilogy of risk . The purpose will be to provide specialist Information, advice, and assistance to the person at the point of contact to reduce escalation of needs as support provided at the right time by the right professional. Through providing Information advice and assistance a proportionate assessment will be completed in collaboration with the family using the model of signs of safety. From this proportionate assessment the Triage will identify next steps. • Such as Making swift, potentially more effective referrals to specialist services for medium to longer-term interventions relating to trilogy of risk issues. • Holding the baton’ in the meantime with family members – doing ‘warm up’ work with them to keep momentum, interest, and supporting small steps of progress. • Working closely with the case holding social worker doing joint visits, providing advice and ‘consultancy’ support, and some direct work with families on trilogy of risk issues. To support the development and delivery of appropriate triage in Porth Gofal. As a specialist domestic abuse support worker you will work with a specialist worker from MIND and DDAS as part of the children and young people’s triage in Porth Gofal. You will complete the front door preventative proportional assessments and undertake visits to develop an appropriate response with families where there is evidence of concern round domestic abuse, mental health and substance abuse. The aim is to develop and complete appropriate safety and support plans round the domestic abuse and to contribute to the recovery plans where substance misuse and mental health is identified to ensure appropriate step down to PCC or relevant agencies and provide support to ensure a coordinated preventative service. The work will include outreach to families assessed and you will be expected to hold a small case load and also to take part in the assessment of families where domestic abuse is present so as to enable survivors to understand the abuse they are subjected to, to hold the abuser to account and identify the effects on children and make appropriate reports for further work with families to address the abuse. The post holder will liaise with the adult and children and young people’s support team of WWDAS to ensure the embedding of learning into the team and to ensure WWDAS effectively delivers its aims and objectives including effective outreach methods, individual casework and group work. MAIN DUTIES AND RESPONSIBILITIES: The postholder is required to work with minimal supervision. They will carry out some or all of the following tasks, depending upon location, qualifications and experience: • Working with the specialist workers in Porth Gofal as the domestic abuse specialist worker you will identify key issues of abuse in families and ensure effective support for women, men and children who are victims of domestic abuse and hold abusers to account. • Day to day work will include making appropriate and proportional assessment of families to prevent risk of escalation, identify appropriate pathways of support with colleagues and with social care in Porth Gofal. • To carry out support meetings with clients as appropriate using outreach work and one to one casework for those who have experienced domestic abuse. • Encourage and promote a multi-agency approach of working to ensure the needs of the service users are met to a high quality which will include partnership working within all aspects of service provision including when relevant, support and safety planning. CLIENTS • To offer appropriate information, advice and support to people in a timely, confidential and professional manner. • To accept and assess referrals and work with service users in accordance with WWDAS Quality Standards and established policies, procedures and guidelines, maintaining all necessary records (manual and computer). • Empower service users to improve their quality of life by promoting and maintaining independent living for themselves and any dependents they may have. • Provide relevant support in accordance with service users’ needs through safe outreach work, one to one support sessions, identifying needs, risks and developing appropriate support and safety plans and case management. • To advise, assist and support service users in accessing services such as housing and social services, helping with welfare benefits, legal services, counselling and other resources such as training and educational opportunities as required. • Enabling service users (survivors) to develop skills required for independent living and their broader needs by enabling access to other relevant services. • Ensuring that abusive and harmful behaviour is identified and the implications for families explored. • To establish close contacts with the various communities and statutory and voluntary organisations in the area. • To inform good practice and policy development on the causes and effects of domestic violence and how this is affected by substance misuse and mental health. • Build effective partnerships with external agencies to support the delivery and promotion of domestic abuse services. You will ensure that you publicise and promote the details of the programme amongst the communities it needs to reach. • To develop robust Service User Inclusion including promoting events, networking, fundraising and consulting with service users to ensure that they are fully involved in service review and development. • Ensure a proactive approach safeguarding and child protection issues and report any such concerns as a matter of urgency. ADMINISTRATION • To maintain accurate and up to date detailed case notes and client records in accordance with Data Protection and other legislation, Ceredigion Council and WWDAS policies and procedures and good practice. • To undertake own routine administration e.g. calendars, word processing, filing, archiving in shared computer systems. • To compile and produce reports including statistics, letters and information packs. • To respond to correspondence, including email, concerning clients. HUMAN RESOURCES • To participate in the mentoring and induction of new staff and volunteers. • To give ongoing support to volunteers and other staff members which includes outreach work. • Under delegation from the line manager, to assist in the sourcing of adequate staff and volunteer rota cover. PUBLIC AWARNESS AND PUBLICATIONS • To provide and maintain relevant and current information materials to meet both clients' needs and those members of the public who are interested in our work. • To be an ambassador of the organisation and identify potential sources of sponsorship and additional income streams. GENERAL: • Observes professional etiquette and ensures confidentiality is kept at all times. • To identify own training and development needs and participate in all training courses and learning activities relevant to West Wales Domestic Abuse Service’s commitment to providing high-quality services. • To work at all times with due regard to the policies and procedures of West Wales Domestic Abuse Service, including financial, data protection and safeguarding regulations, participating in their development and amendment where required. • Willingness to work outside normal working hours including evenings and weekends and to participate in on call duties as well as promotional, fundraising and income generating events, activities and any other duties as may be reasonably required by the organisation. PERSON SPECIFICATION CRITERIA ESSENTIAL/ DESIRABLE ASSESSED EDUCATION/QUALIFICATION 1. 5 GCSE’s Grade A-C including Math and English or equivalent. EXPERIENCE 2. Experience of working with individuals who have experienced domestic abuse 3. Recent experience of case management of clients. 4. Experience of signposting service users to other agencies for support specific to their needs 5. Experience of training and or giving group presentations to a variety of audiences 6. Experience of working with clients with substance misuse or mental health issues KNOWLEDGE AND UNDERSTANDING 7. An excellent level of understanding of the impact of domestic abuse on individuals and families. 8. An understanding and appreciation of the additional difficulties faced women, men and young people who are experiencing domestic abuse across the gender spectrum and how to support and signpost them appropriately. 9. Understanding and awareness of safeguarding and child protection. 10. Good understanding of confidentiality and appropriate personal/professional boundaries. 11. Understanding of working with service users with complex needs associated with mental health, drugs and/or alcohol.
