Job overview

Caravelle ward including Stroke Rehab is a 28 bedded rehab unit with 10 stroke beds and 18 general rehab beds. We provide a tailored rehabilitation programme to meet the individual patients needs and help them regain maximum function through realistic goal setting.

We have a multi-disciplinary team approach ensuring that patient and their family are included in care decisions from the first day of admission.

We are looking for staff nurse to join our team. Knowledge of inpatient rehabilitation would be useful.

Main duties of the job

We are a team of RNs, Advanced Clinical Practitioners, Healthcare assistants, Physiotherapists, Occupational Therapists , Rehabilitation support workers, Speech and Language Therapist and Ward Doctor who aim to maximise our patients’ potential to enable them to return home. We have a visiting Stroke Consultant and a Geriatric Consultant both from an acute hospital who see to and review stroke and medical patients respectively once a week. When that is not possible we work closely with our adult social care colleagues to plan best outcomes.

We are looking for an RN who enjoys working as part of a friendly and supportive team. An RN who applies the importance of prioritization and leading the members in their team assertively. It is hard work but very rewarding – planning the patient pathway, enabling them to progress and supporting them through setbacks is what we do best.

As this is a 24 hour service, internal rotation will be operating.

To deliver multi-professional high quality care, 24 hours a day, working with patients towards rehabilitation, self-management and independence throughout the continuum of care. To lead a team of nurses supporting evidence based care, facilitating staff learning and development.
