Service Coordinator

As a Service Coordinator, you'll be at the heart of our patient support work - making settling-in calls to people recently discharged from hospital, ensuring they feel safe, supported, and well-informed. You'll gather valuable insights to help improve health and care services, while connecting people to the right support through referrals and signposting. Working closely with hospital teams and colleagues, you'll help deliver a seamless, compassionate service that truly makes a difference.

  • Responsible for building strong positive relationships and liaising with hospital staff and other medical professionals in the community, including GP surgeries, to receive patient referrals.
  • Managing a team of staff and volunteers.
  • Assigning incoming referrals to support workers. Evaluating and analysing the outcomes of the services.
  • Monitoring the service delivery, ensuring service health and safety and evaluating progress.
  • Overseeing record-keeping and recruiting.

What does it take to be a successful Service Coordinator? You’ll need:

  • Strong communication skills with a professional, diplomatic, and confidential manner.
  • A good understanding of how to improve service quality for the benefit of users.
  • The ability to work collaboratively and bring people together across teams and services.
  • Confidence using IT systems to manage referrals and record outcomes.
  • A full UK driving licence and access to a vehicle.
  • A flexible approach to working hours to meet service needs.
