POST: TERM: SALARY RANGE: HOURS: QUALIFICATIONS: Clinical Pharmacist - Independent Prescriber- JOB DESCRIPTION Clinical Pharmacist (independent prescriber) Fixed term – 6 months £39,000 – £44,500 pro rata Part-time 12 hours per week, 9.00am-5.30pm – flexible hours. Relevant degree in pharmacy Registration with the General Pharmaceutical Council Independent prescriber qualification ACCOUNTABLE TO: Medical Director / Senior Research Nurse JOB SUMMARY: You will be part of a multi-disciplinary team (doctors, nurses and psychologists) working across a range of clinical & research activities at ReMind UK. You will be working in a patient-facing role to support the memory clinic team with the prescribing of appropriate medication for these patients. GENERAL ReMind UK is an independent charity and an internationally renowned centre for research, diagnosis and treatment of neurodegenerative diseases. Our research aims to improve life for people with dementia and their families and carers, and to find drug and non-drug treatments for people with conditions such as Alzheimer’s disease and other diseases predominantly affecting older people. Our contracted NHS Memory Clinic service, paid for memory assessments, and allied activities also provide support to patients, families and carers. We are currently in an ambitious phase of development to increase significantly our clinical and academic research programme. We work closely with the Universities of Bath, Bristol and other research institutions, both nationally and internationally, and with the Royal United Hospital. THE ROLE The role will involve monitoring our patients with Alzheimer’s disease or dementia with Lewy bodies who have been commenced on medication, to oversee medication titration and stabilization, and to provide advice regarding the choice of alternative medication where necessary. Based at ReMind UK, this will primarily be via telephone consultations with our varied patient group, who often also have polypharmacy and significant co- morbidity. You will be liaising with patients, their carers and their community health care team. You will be supported by our friendly team of medical and nursing experts. CLINICAL RESPONSIBILITIES • Undertake medication reviews for patients who have been commenced on dementia medication and are requesting repeat prescriptions • Oversee medication titration and stabilization before handing prescribing over to the primary healthcare team according to ReMind UK protocols • Answer patient queries and concerns about their medication and any possible side effects, including changing the prescription where necessary ReMind UK, The Research Institute for Brain Health, Royal United Hospital, Combe Park, Bath BA1 3NG Reception +44 (0) 1225 476 420 · www.reminduk.org · info@reminduk.org ReMind UK is the trading name for RICE - The Research Institute for the Care of Older People Registered Charity no: 1042559 · Company Ltd by Guarantee no: 2979617. • Prescribing in a cost-effective manner • Advising the primary healthcare teams on options for deprescribing or substitution recommendations if there are medications affecting their cognition • Liaising with the primary healthcare teams regarding patient contacts and any other changes to medication dose • Maintaining excellent medical records.
