JOB SUMMARY:

You will be part of a multi-disciplinary team (doctors, nurses and
psychologists)  working  across  a  range  of  clinical  &  research
activities at ReMind UK. You will be working in a patient-facing role
to  support  the  memory  clinic  team  with  the  prescribing  of
appropriate medication for these patients.

GENERAL
ReMind  UK  is  an  independent  charity  and  an  internationally  renowned  centre  for  research,  diagnosis  and
treatment of neurodegenerative diseases. Our research aims to improve life for people with dementia and their
families and carers, and to find drug and non-drug treatments for people with conditions such as Alzheimer’s
disease and other diseases predominantly affecting older people. Our contracted NHS Memory Clinic service,
paid for memory assessments, and allied activities also provide support to patients, families and carers. We are
currently  in  an ambitious  phase of  development to  increase significantly  our  clinical  and  academic research
programme. We work closely with the Universities of Bath, Bristol and other research institutions, both nationally
and internationally, and with the Royal United Hospital.

THE ROLE
The role will involve monitoring our patients with Alzheimer’s disease or dementia with Lewy bodies who have
been commenced on medication, to oversee medication titration and stabilization, and to provide advice
regarding the choice of alternative medication where necessary. Based at ReMind UK, this will primarily be via
telephone consultations with our varied patient group, who often also have polypharmacy and significant co-
morbidity. You will be liaising with patients, their carers and their community health care team. You will be
supported by our friendly team of medical and nursing experts.

CLINICAL RESPONSIBILITIES

• Undertake medication reviews for patients who have been commenced on dementia medication and are

requesting repeat prescriptions

• Oversee medication titration and stabilization before handing prescribing over to the primary healthcare

team according to ReMind UK protocols

• Answer patient queries and concerns about their medication and any possible side effects, including

changing the prescription where necessary

•  Prescribing in a cost-effective manner
•  Advising the primary healthcare teams on options for deprescribing or substitution recommendations if

there are medications affecting their cognition

•  Liaising with the primary healthcare teams regarding patient contacts and any other changes to medication

dose

•  Maintaining excellent medical records.

PROFESSIONAL & EDUCATIONAL RESPONSIBILITIES:
•  Abide by all legal and statutory regulations relating to practice
•  Comply with the legal requirements and rights of the individual according to Mental Health legislation
•  Maintain standards in accordance with the General Pharmaceutical Council
•  Be familiar with all ReMind UK policies and Standard Operating Procedures (SOPs) and comply with these at

all times, and assist in ensuring the SOPs are kept up to date

•  Help to ensure that patients and carers are as fully informed as possible about their illnesses and benefits

to which they are entitled

•  Willingness to help with and give presentations at support courses
•  Promote the work of ReMind UK to visitors, including other health professionals and members of the

public, and represent ReMind UK at external functions

•  Take responsibility for on-going personal development and education
•  Share skills and contribute to training new nursing, medical and other staff
•  Contribute to educational sessions, eg Journal Club

OTHER RESPONSIBILITIES
•  All staff are required to carry out other such duties as may reasonably be required for the smooth running of

ReMind UK

•  All ReMind UK staff must be eligible to work in the United Kingdom
•  All staff are required to undergo DBS checks, and to sign a confidentiality agreement

PERSON SPECIFICATION

ESSENTIAL

Qualifications and Experience

•  Relevant Pharmaceutical degree (MPharma)
•  Registration as a pharmacist with the General Pharmaceutical Council
•

Independent prescriber qualification (GPhC)

•  Excellent interpersonal skills
•  Proven empathetic and caring approach to patients and relatives, and assured patient confidentiality, dignity

and respect at all times

•  Ability to prioritise own busy workload and to work under own initiative within boundaries
•  Demonstrate awareness and importance of flexibility and working as part of a team
•  Knowledge of the structure and workings of the primary healthcare team
