Due to an exciting expansion of services, the hospice is adding to their well-established medical team.

We are seeking to recruit an experienced advanced nurse practitioner with a passion for delivering exemplary care, to join us in looking after people affected by life-limiting illness in both the hospice and community settings.

You will be based in our purpose-built hospice situated one mile from Macclesfield town centre. There is ample free parking available on site, and subsidised meals are available.

Our service will operate 7 days a week, between the hours of 8am and 8pm.

Our new team member will:

• Undertake face to face reviews of palliative patients, working across clinical settings, inclusive of hospice & community work
• Deliver expert advice to healthcare professionals, patients and relatives both in person and over the phone
• Coordinate care for people with a life-limiting illness
• Be confident liaising with other healthcare professionals, making recommendations for changes to medication regimes/plans of care and advocating for patients and their families
• Be proactive in-service development
• Deliver evidence based, quality care
