Community Health Care Assistant 25 hours 

This exciting role to work as a Community health Care Assistant within the multidisciplinary team working across Middlesbrough Greater PCN.

This is an opportunity for those who are interested in working in the local community and being part of an exciting new model of working in the community localities of Middlesbrough.

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

Main duties of the job

To work as part of the community nursing team; we deliver a 7 day service , including some weekends.

The post holder will undertake a broad range of care activities, including wound care and venepuncture

The role will require some flexibility and this may require the post holder to work across acute and community in response to service demands.

To support the delivery of patient care and other duties that contributes to the care environment. This might include, for example, having a “Link” role; supporting therapy and nursing/medical staff.

To support the delivery of a range of duties that support patient care and maintain a suitable working environment.

Knowledge & Skills

Essential criteria
  • Excellent interpersonal, communication and active listening skills. Ability to adopt appropriate styles and methods of communication.
  • Basic IT skills to use electronic patient records and other Trust systems.
  • Ability to demonstrate the importance of holistic person-centred care and related procedures
  • Understanding of the importance of robust, accurate, clear, timely documentation
  • Attention to detail.

Personal Attributes

  • Access to a vehicle for work purposes to meet the requirements of the role

Experience

Essential criteria
  • Experience of working in health or social care
  • Experience of undertaking and recording clinical observations and escalation of changes or findings.

Qualifications

Essential criteria
  • Numeracy and Literacy Level 2 or • Maths & English GCSE A-C/4-9 or • Maths & English O Level/GCE A-C or • Maths & English CSE Grade 1 or • NARIC/ENIC which compares the qualification to UK GCSE (https://www.enic-naric.net/)
  • Care Certificate
  • Level 3 vocational qualification or • Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties and/or • Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3. - Theoretical knowledge - Experience of a range of care and related procedures - clinical observations - knowledge of relevant legislation
