The role
As we continue to evolve our services to meet the needs of our community, we are looking for three Senior Staff Nurses to join our Integrated Community Service.
The post holders will be responsive, flexible and able to provide high-quality person-centred specialist palliative care to patients and those important to them in the community, within the Hospice at Home team or the Advice and Guidance team.

You will:
- Provide specialist complex palliative nursing care in the last months, weeks and day of life to patients in the community.
- Complete the patient triage process either face to face or on the telephone
- Be flexible in your approach to assessing patients, completing this either in the hospice or in the patient’s home
- Complete assessments of patients referred to Integrated Community Service at any stage in their palliative diagnosis.
- Transfer patients care to other hospice services as appropriate when nursing interventions are no longer required, or additional MDT input is required to support the patient.
- Work independently during weekends and as part of a 7-day rota, including bank holidays.
- Lead and coordinate care planning, ensuring safe, effective, and personalised care.
- Delegate appropriately and support the work of Senior Healthcare Assistants (SHCAs)that work within the Hospice at Home team.
- Collaborate closely with the wider multi-disciplinary teams (MDT)that form the Integrated Community Services.
About you
We are seeking three skilled and experienced nurses who are:
- Ideally experienced in community nursing and/or palliative care.
- Resilient and confident to work independently and autonomously, including lone working at weekends.
- Committed to developing professionallywithin a supportive and values-led environment.
- Skilled at delegating tasksand coordinating care across a team.
- A strong communicator with the ability to work collaboratively with an MDT.
- Driving licence and access to a vehicle are essential for work purposes.
