JOB DESCRIPTION TITLE OF JOB: Ageing Well Support Coordinator JOB REFERENCE: AWSC/Aug25 SERVICE: Ageing Well CLOSING DATE FOR APPLICATIONS: 02/10/25 INTERVIEW DATE: To be confirmed HOURS PER WEEK: 18 TEMPORARY/PERMANENT: Temporary - maternity cover SALARY: Band 3, Scale Point 13, starting £26,994 FTE (i.e. £13,132 actual) paid monthly by Automated Credit Transfer on 25th of each month HOLIDAY ENTITLEMENT: 27 days pro rata per annum, plus 3 extra days at Christmas and the usual statutory holidays. ACCOUNTABLE TO: Ageing Well Information, Help and Advice Team Leader LINE MANAGED BY: Ageing Well Information, Help and Advice Team Leader RESPONSIBLE FOR: • Respond to telephone and email enquiries coming into the Ageing Well IHAT service. Taking a lead for information queries linked to groups and activities and promotion of health • Working collaborative with the team to ensure high-quality service delivery. • Building and maintaining relationships with key Ageing Well partners, community organisations, and referral agencies. • Working with the Team Leader to develop and implement service improvements based on feedback, performance data, and emerging needs in the community. • Co-ordinating IHATs promotional activities, training initiatives, and community engagement to raise awareness of the Ageing Well service. Service Coordination and Improvement: • Ensuring that Older People, their carers, and professionals receive high-quality, accurate, and timely information. • Work with the team to improve how the service runs, including updating information systems and ways we collect feedback. • Ensure accurate recording of all contact enquiries, data reporting, and follow-up actions to measure impact and outcomes. 2. Team Support: • Collaboratively working with colleagues, ensuring a supportive and high- performance working environment. • Assist in the induction of new team members and volunteers to effectively deliver information and support services. • Fully participate in regular check-ins and team meetings to review challenges, share best practices, and identify areas for improvement. 3. Stakeholder Engagement and Partnership Working: • Support existing relationships with local health, voluntary and statutory organisations, helping to maintain communication and coordination. • Represent the Ageing Well service in external meetings, forums, and networking events to advocate for the needs of Older People. • Work with referral agencies to improve pathways for Older People to access appropriate services. 4. Communications and Outreach: • Work with the Communication and Marketing Assistant to promote Ageing Well and the health promotion campaigns in the city, including using social media, printed materials, and networking opportunities. • Organise and participate in community events, health campaigns, and outreach activities to increase awareness and engagement. • Support the IHAT team in developing presentations to community groups and professionals on Ageing Well services, where needed. 5. Operational Support: • Provide occasional cover for food access services and other related Ageing Well projects as needed. • Support the delivery of the annual Ageing Well Festival with planning, scheduling, coordinating bookings, promotion, and use of planning tools. • Assist IHAT Team with producing the annual Festive Directory.
