The Forward Trust is looking for a dedicated RECONNECT Worker to deliver a comprehensive, inclusive, and bespoke support service to clients preparing for life after prison.
In this role, you will be working across HMP Wayland, Norwich, and Bure, helping service users access physical and mental health support, general well-being services, and wider community connections to ensure a smooth transition back into society.
What you’ll do:
- Deliver tailored advice and guidance to Forward Trust clients across HMP Wayland, Norwich, and Bure.
- Conduct in-depth assessments to identify client needs 12 weeks prior to release.
- Work in partnership with multiple agencies to create bespoke care plans with the client at the centre.
- Provide ongoing support for up to 6 months post-release, including mentor link-ins, GP referrals, aftercare, and signposting.
- Maintain excellent relationships with external agencies and key stakeholders.
- Implement and maintain clear referral pathways, including Offender Management in Custody, Family/Self, Prison, and Healthcare.
- Ensure a holistic service is delivered safely and effectively, in line with individual and team targets.
- Collaborate with colleagues to share best practices and fully engage with risk assessment processes.
- Help service users connect with local services and the wider Forward Trust community to support sustainable reintegration.
What we’re looking for:
You’ll be someone who thrives in a collaborative environment, can manage multiple priorities, and is committed to making a real difference in people’s lives. Experience working in criminal justice, mental health, or social care is desirable, as is excellent communication and relationship-building skills.
