Job Summary

Health care assistant for the Inpatient unit and Community Services including
Hospice at Home, Wellbeing and Outpatients

Key Responsibilities
Clinical

•  Assist the registered health practitioners in developing and maintaining a safe,

caring and supportive environment for patients, relatives and staff.

•  To assist and work under the instruction of a designated person to deliver care

to patients on the inpatient unit and in community services.

o  Carry out personal care including washing and dressing patients as per

care plan

o  Support patients with their nutritional needs and simple meal

preparation with supervision

o  Support patients with their mobility

•  Communicate clearly and effectively with patients, families and other health

care professionals

•  Use electronic systems to document patient care with support and

supervision

•  Share written information with families and patients as directed
•  To work alongside the clinical team, engaging in clinical handovers, safety

huddles and team meetings

•  Upholding all infection prevention and control requirements
•  Services run 7 days a week with shift work covering long days and short shifts

and nights. (Day shifts only whilst undertaking apprenticeship)

•  The ability to drive and have access to a car is required for this role, therefore a
full driving license or willingness to learn how to drive is essential for this role

 Training & professional development

•  To achieve training and development requirements in line with apprenticeship

learning programme and as identified in annual appraisal process.
•  To attend all sessions associated with apprenticeship programme, meet

deadlines for assignments and assessments.

•  To raise any concerns about progress in apprenticeship programme with

Clinical services manager/community services manager

•  To complete and maintain all mandatory training
•  To attend all internal education sessions as directed.

patients, members of staff or other hospice business. On no account must information
relating  to  identifiable  patients  be  divulged  to  anyone  other  than  those  authorised
persons, for example, medical, nursing or other professional staff, as appropriate, who
are concerned directly with the care, diagnosis and or/treatment of the patient. If you
are  in  any  doubt  whatsoever  as  to  the  authority  of  a  person  or  body  asking  for
information  of  this  nature,  you  must  seek  advice  from  your  manager.  Similarly,  no
information  of  a  personal  or  confidential  nature  concerning  individual  members  of
staff should be divulged to anyone without the proper authority having first been given.

General Data Protection Regulations (GDPR)
Wirral Hospice St John’s is required to process personal data relating to its employees,
including  ‘special  categories  of  personal  data’,  as  defined  in  the  General  Data
Protection Regulations 2018 (the ‘Act’).

All such data will be processed in accordance with the provisions of the Act and the
relevant hospice’s policies. For the purposes of the Act, the term ‘processing’ includes
the  initial  collection  of  personal  data,  the  holding  and  use  of  such  data,  as  well  as
access and disclosure, through to final destruction.
