Job overview

Are you an experienced, enthusiastic Advanced Clinical Practitioner (ACP) looking for a new career opportunity in Primary Care? If so, an exciting opportunity has arisen for you to join one of our dynamic GP practices in Morpeth.

We are looking for a highly motivated, enthusiastic team player who can demonstrate high standards of clinical excellence. You will become part of an experienced, pro-active and friendly team encompassing GP's, Practice Nurses and Nursing Associates, and Admin Staff. You will help assist the current practice team in delivering gold standard patient care through general practice.

Due to the nature of this role, it is an essential requirement of the role that the post holder has a valid driving license and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role.

The role will involve working across sites and covering extended access provision.

Main duties of the job

As an ACP, you will play a vital role in the assessment and delivery of excellent care to patients with acute and chronic conditions.

You will have significant, relevant experience working as an autonomous practitioner. Triaging patients effectively, providing over the telephone advice and support or welcoming patients into the surgery for an appointment with an appropriate clinician. You will also support the practice with the care they provide to the local Care Home and complete home visits when required.

As a senior member of the team, you will provide leadership and support within the multi-professional team. You will support the wider team to build capacity and capability and act as a role model, educator, supervisor, coach and mentor. You will supervise junior members of the team and assist with their development. You will also support the Practice Lead in appraising junior members of the nursing team.

You will contribute to the work of the Health Centre through the provision of quality evidenced based care and the appropriate use and deployment of resources. This includes providing autonomous critical thinking and diagnostic reasoning when making clinical decisions. You will undertake the management of patients over a period of time and organise appropriate services, as well as delivering direct patient care.

Detailed job description and main responsibilities

The post holder will be expected to manage their own clinical workload participating in both face to face and telephone consultations and making professional autonomous decisions. The post holder will be included in the clinical rota which includes on-call sessions.

The post holder will support the medical and nursing teams.

The practitioner will-

  • Use advanced knowledge and skills in to undertake comprehensive clinical assessments, make decisions based on clinical reasoning and initiate, evaluate and modify a range of interventions
  • Provide professional senior clinical leadership and support within the specialty multi professional team and across service boundaries

You will support the wider team to build capacity and capability through work-based and interprofessional learning, and the application of learning to practice. You'll act as a role model, educator, supervisor, coach and mentor.

Qualifications

Essential criteria
  • Professional registration with NMC / HCPC / GPC
  • Relevant degree, minimum 2:2
  • Relevant Master’s degree
  • Prescribing qualification
  • Evidence of relevant, recent study continuing education.
  • Registered Mentor
Desirable criteria
  • Doctoral or PHD degree or working towards
  • Management and leadership qualification
  • ALS / APLS
  • Major Incident Training

Experience and knowledge

Essential criteria
  • Extensive demonstrable senior experience in specialty / clinical area
  • Experience of implementing evidence based practice
  • Experience of participation in clinical audit
  • Evidence of management ability and leadership qualities.
  • Evidence of ability to work autonomously within a variety of clinical situations.
  • Knowledge of or experience in coaching and mentoring practices and tools
  • Knowledge of or experience in Quality improvement tools, techniques and methods
Desirable criteria
  • Experience of leading clinical governance
  • Experience of research activity
  • Experience of Service Improvement / Quality Improvement / Practice Development
  • Experience of developing evidence based protocols and clinical guidelines.
  • On call experience
  • Evidence of supporting other clinicians
