Working hours: Mon, Tue, Thur, Fri - 7am – 10:45 and/or 2:30 – 6:15pm. This can be a job share - 30 hours per week or 15 hours per week if a job share.
SCCCC has over 50 years’ experience in providing services that respond to the needs of vulnerable people in a practical and neighbourly way, complementing the care given by other service providers. SCCCC work in partnership with Sheffield Teaching Hospitals, Sheffield City Council and Sheffield Integrated Care Board.

The role
An exciting opportunity has arisen to be part of a brand-new service being launched in collaboration with St Lukes.
This service person centred service will support palliative and End of Life patients to and from St Lukes day centres and to facilitate patients to be discharged from Hospice beds back to their own home or to transport patients to hospice beds
We are looking for a caring, compassionate, and self-motivated individual to join our team to support the implementation of this exciting new service.
Playing a pivotal role in providing excellent support to older and vulnerable people who need it, you will have a flexible approach and be able to adapt to changing circumstances as they arise as the role may see you supporting family members with there concerns as well as patients. Drivers will be supported on journeys by a St Lukes team member.

How the Service Works
Professionals from St Luke’s will identify patients that would benefit from this service and will contact you to arrange logistics. Patients and or family/carers/friends will be contacted by you to arrange pick up and drop off times.
Duties and Responsibilities
- Liaise with St Luke’s team members to screen referrals for transportation, in the team leads absence
- To work as part of a team with the Passenger Assistant to undertake the safe carriage of passengers to various locations using a variety of vehicles carrying up to 16 passengers.
- To assist passengers with their mobility aids into and out of vehicles if necessary.
- To undertake loading and unloading duties related to passengers and equipment being transported.
- To operate the tail lift to load passengers in wheelchairs into the vehicle and to undertake the safe clamping of wheelchairs when driving Transporters.
- To carry out and record daily vehicle checks, fuelling and cleaning duties to specific standards.
- To report any vehicle defects immediately to Team Lead
- To assist the Passenger Assistant in any emergency situation that may arise.
- To plan the routes and carry out journeys in the most economic and efficient way.
- To report any accidents immediately to the Operational Lead.
- Liaise with Line Manager to ensure seamless service delivery
- Deliver a flexible Individual-centred service
- Provide support to informal carers
- Be available for enquiries from service users and their families/carers/friends and respond to any requests for information in a timely and professional manner.
- Complete a check list to ensure all identified needs have been actioned.
- As appropriate, provide telephone support to service users their families/carers/friends.
- As directed provide ongoing support and supervision to volunteers within SCCCC and ensure that the service is responsive to the needs of its volunteers.
- Ensure that all duties and functions are carried out in accordance with SCCCC’s regulations, policies, and procedures.
- Maintain appropriate records of work undertaken and produce written reports as required.
- Attend the meetings of the organisation and any other meetings as required.
- Attend appropriate training courses to enhance and develop her/his own skills.
- Liaise with the fundraiser in order to maximise donations to the organisation.
- Be an ambassador for SCCCC when out in the community
- Publicise SCCCC schemes through all appropriate channels.
