Trust Links is expanding our mental health services across Mid and South Essex and we’re looking for dedicated, compassionate individuals to join our new Healthy Mind Hubs team.

About the Healthy Mind Hubs

Building on years of successful delivery of Wellbeing Hubs and Recovery College in South East Essex, our new Healthy Mind Hubs will provide holistic, activity-based group support, peer support, and multi-agency collaboration – helping people in Primary Care with emerging mental health needs and preventing escalation to secondary services.

We’re working in partnership with Mind, who will provide 1:1 support, while Trust Links leads on group facilitation, peer development, and community connection.

POST TITLE: Healthy Mind Hub Project Worker

POST SALARY: £24,000 – £26,000 per annum pro rata

HOURS OF WORK: Full or part time roles may be available including evenings and weekend work.

POST LOCATION: Trust Links and external sites across Mid (Chelmsford, Maldon and Braintree) or South West (Thurrock, Brentwood and Basildon) Essex. Travel required across all Trust Links sites including South East Essex (Southend, Castle Point and Rochford) (plus opportunity for working from home)

REPORTS TO: Healthy Mind Hub Area Lead

RESPONSIBLE FOR: Volunteers and Peers

PURPOSE OF THE POST:

Trust Links grows communities and transforms lives. Through our seven Growing Together community gardens across Essex, Healthy Mind Hubs, MSE Recovery College, Trust Links Counselling (TLC), children and youth projects, and environmental workshops we work with more than 4,000 people each year. We work in a person-centred, grassroots and recovery-focused way, helping to save lives every day and move people on to better lives, with the support of their peers and the wider community.

The Healthy Mind Hubs provides holistic wellbeing activity-based group support, peer support and multiagency service delivery throughout Mid and South Essex, with the aim of preventing and reducing the demand on secondary mental health services. The Healthy Mind Hub works closely with the MSE Recovery College, Healthy Mind Community Connectors (employed by Mind), Growing Together, and services run by other organisations including Health Trainers, Citizens Advice, Advocacy and drug and alcohol support services.

Delivery will be seven days per week and staff will be scheduled in advance to deliver on Saturdays and Sundays throughout the year.

MAIN DUTIES AND RESPONSIBILITIES OF THE POST:

· Plan, develop, facilitate and provide group support for adults with mental health support needs at the Healthy Mind Hubs.

· Deliver holistic initial assessments, risk assessments, and goal setting through coaching

· Capture impact using the agreed outcomes tools and case studies

· Attend Multi-Disciplinary Teams, Trust Links sites and other community venues as agreed and scheduled to promote the services and respond to the needs of those presenting to the service

· Support the development of peer support groups and activities

· Liaise with other agencies and partner services, referring and signposting to services

· Collate a portfolio of agencies and resources in the designated area, to be shared with the team, volunteers and members as appropriate

· Record all significant contacts on CRM Charitylog

· Devise, plan and deliver training courses and workshops within areas of knowledge

· Work closely within a multi disciplinary team; attending meetings, giving feedback and communicating with other members of the Healthy Mind Hubs and Trust Links staff

· Work with other professionals external to Trust Links as required

· Proactively promote the work of the Healthy Mind Hubs and other Trust Links services to partners and the public

· Provide supervision and support to volunteers and peer supporters as appropriate

· Ensure that volunteers are fully aware of their responsibilities and have access to and understand Trust Links Policy and Procedure

· Ensure Safeguarding and Health and Safety requirements are met line with Trust Links Safeguarding and Health and Safety Policies and escalate concerns about the well-being of service users to the appropriate agency

· Ensure that services offered are accessible to all service users of the local community, promoting an inclusive approach

· Ensure that service users are able to contribute fully to the design, delivery and evaluation of services, ensuring services respond to local need

· Participate in regular supervision with the Healthy Mind Hub Management and take responsibility for continuing self development, participating in training and development activities as appropriate

· Any other duties as required by the line management commensurate with the post

PERSON SPECIFICATION:

Essential

· Experience working with adults with mental health problems in the statutory or voluntary sector

· Person centred, with a flexible approach in meeting client-centred objectives

· Experience of facilitating and supporting positive peer support activities

· Good documentation and report writing skills and effective verbal communication

· Excellent work ethic, team player and good interpersonal skills

· Ability to prioritise, work under pressure and organise time to meet project targets

· Experience of multi-disciplinary and multi-agency working

· Ability to bring about change

· Willing to undergo DBS check

· Excellent communication and people skills

· Ability to build networks and make connections
