Dementia Wellbeing Connector Carmarthenshire

Salary: DP3(1)- £25,298

Hours: 35 hours per week.

Place of Work: Hybrid working model from home and Llanelli office. Travel
throughout Carmarthenshire to client homes.

Contract: Fixed term contract to 31/5/2027 with possible extension, subject to further
funding.

We are looking to recruit a self-motivated, enthusiastic, hard-working team member to
join our exciting new service that will support people living with Dementia (PLWD)
and their carers. The service aims to provide a full wrap-around support that will
assist the PLWD in navigating their individual journey and increase their overall
wellbeing as much as possible.
