Job Description RE:Start Team Leader (West) Post: RE:Start Team Leader (West) Responsible to: Lowdown CEO (Liaising directly with RE:Start Manager) Responsible for: West Area Support Workers and Youth Engagement Worker Work location: Based in Northampton, but covering West Northants Salary range: £34,000 Contract: Permanent Hours: 37 hours a week Flexible working to meet the requirement of the role. Will include evening and weekend hours. About The Reach Collaborative The REACH Collaborative consists of five highly regarded agencies who offer free, professional, and confidential support services for young people aged 5-25 across North and West Northamptonshire. The Collaborative has been commissioned to support young people aged 16 to 25 transitioning from child and adolescent to adult mental health services. 16 – 25 RE:Start Transition Team Youth Works, on behalf of the REACH Collaborative have been commissioned by NHS Northamptonshire to deliver the exciting RE:Start service to support young people making the transition from Child and Adolescent Mental Health Services to Adult Mental Health Services. RE:Start is exclusively for 16 – 25-year-olds, who live in Northamptonshire, and need additional support with navigating mental health services. This can be a difficult and challenging time for many young people. Support will be offered to young people via a referral system. The RE:Start service will introduce young adults to the comprehensive crisis pathway available across Northamptonshire, and will ensure co-produced, personalised Safety Plans for each young adult, to help avoid unnecessary attendance at emergency departments. Progress will be monitored through an agreed outcome measure. The Service will use a 'Personal Assistant (PA)' approach to ensure a flexible, holistic approach to care. It will have the capacity to work with young people from 16 years old building trust and gaining a robust understanding of needs - and to remain involved with young adults up to the age of 25 to ensure a gradual transition into adulthood offering practical and emotional support. The ethos and culture of the service will be solution-focused, strengths-based, personalised, flexible and will be able to work with a complex set of needs, regardless of diagnosis. There are two teams of support workers, one in the North of the county based at Youth Works and one in the West based at The Lowdown. Job Purpose To lead a team of support workers in delivering and developing the RE: Start transitions service to young people in the West of the county. General Duties 1. Lead on the development and delivery of a range of quality support services including assessment, casework, workshops and regular social group activities and signposting based on the presenting needs of the client group in the West. 2. Provide appropriate line management to a team of 4 support workers plus a youth engagement worker. 3. Ensure a clear referral pathway into the service is maintained and regularly reviewed and that clients are contacted within given timeframes. 4. Provide initial assessment and casework support to a caseload of clients. 5. Ensure personalised safety plans are completed for all clients. 6. A member of the Lowdown’s Safeguarding Team and responsible for overseeing safeguarding concerns and supporting the team regarding risk management. 7. Adopt a person-centred approach to work, based on the I Thrive and trauma centred principles. 8. Offer ongoing support with related issues e.g. housing, finances, access to education, employment or training, socialization and community support, support with any drug or alcohol issues through direct support and signposting. Oversee the development of a local database of agencies regarding signposting. 9. Ensure all monitoring outcomes data is recorded and input. 10. Collate and present monthly statistical reports and provide any written reports, case studies and summaries of service delivery to RE: Start Manager to enable 3 monthly evaluation reports to be produced for the service. 11. Support and ensure the input of users into the service development through a Youth involvement user group. 12. Lead and participate in regular team meetings and ensure that team meeting minutes are read and acted upon. 13. Ensure that both teams come together and share practice and specialised information and support through county briefings and collaborations. 14. Collaborate effectively with GPs, local NHS providers (e.g. CAMHS etc.) plus other statutory and voluntary organisations to receive and make appropriate referrals. Some flexibility in role will be required in the role for the service to adapt and develop. RE:Start Team Leader (West) Person Specification Essential criteria Desirable criteria Education /Knowledge Skills A professional qualification in a related field e.g. social work, youth work, teaching, counselling, IAG and/or a BA Hons degree Training in management/first line supervision Management/supervisory skills in work with young people Skills in counselling and/or emotional support or case work with young people/young adults Good time management and organisational skills Experience in a related field (e.g. housing, addictions, employment, benefits. Able to produce monitoring data and reports and to develop monitoring systems to meet need Ability to influence practice through partnership working and liaison with other agencies Experience Computer literacy and inputting data Knowledge of Microsoft/Google suite of products Ability to recognise and meet the individual needs of people who use our services through both face-to- face work and the effective management of others. At least one years’ experience of managing/supervising staff and/or volunteers in a supportive and collaborative way.
