Job overview

An exciting opportunity is available to join the all age continuing care service covering the North East and North Cumbria as a children's continuing care (CCC) nurse assessor/case officer to potentially to cover the North Cumbria locality. The service undertakes assessments and commissions packages of care across the North East and North Cumbria in line with the National Frameworks for Continuing Healthcare and Children's Continuing Care. Main duties of the job

The CCC nurse assessor/case officer is a key role in the CCC service meeting the unmet health needs of children and young people in the NENC area. The CCC team are keen to hear from qualified nurses with experience of undertaking assessments for children and young people with a variety of health needs especially mental health and or learning disability.

The ICB is responsible for ensuring that high quality and safe health services are accessible to all of our communities. It has a wide range of functions, including; promoting integration of health and care services, improving peoples health and well-being, reducing health inequalities.

With a £6.6 billion budget and a workforce of 170,000 people across the North East and North Cumbria. As well as strategic functions, ICB staff also work at place level with local health and wellbeing boards in each of our 14 local authority areas. These teams also work alongside our 67 primary care networks which are groups of local GP practices, social care teams and other community- based area providers.

The ICB works with our Provider Collaborative too, this includes the 11 NHS Foundation Trusts in the region, to deliver our shared priorities.

Detailed job description and main responsibilities

This role will be responsible for:

•    Clinical assessments of the health needs of children and young people to determine eligibility for CCC funding. 
•    Coordinating and completing MDT assessments and reviews in accordance with the National Framework.
•    Provide a high standard of holistic health assessments within the agreed timeframes. 
•    Commissioning of the packages of care to meet the unmet health needs of children and young people eligible for CCC.
•    Supporting children and young people with the offer of a personal health budget to meet the health outcomes. 
•    Case management for children and young people eligible for funding via CCC.
•    Record keeping as per NENC policy and procedures. 
•    Working in partnership with children, young people, and their representatives. 
•    Building and maintain relationships with Local Authorities, providers, Foundation Trusts, and the voluntary sector
•    Contributing to the ICB's achievement of their statutory duties.
•    Supporting the transition of young people to adulthood. 
•    Contributing to the ICB statutory duties for SEND.

Qualifications

Essential criteria
  • First Level Registration NMC – RSCN, degree or relevant experience
Desirable criteria
  • Qualification in children’s health care or Learning Disabilities

Experience

Essential criteria
  • Experience of clinical audit
  • Experience of completing multi agency assessments
  • Previous experience of working within an extremely busy office environment
  • Experience in working within a multi-disciplinary team setting
Desirable criteria
  • Prior experience in Continuing Care
