Contract: Part Time

Salary: £13,111.80

To work as part of a multi- disciplinary team on the Wellbeing unit, delivering high-quality, evidence-based holistic palliative care to patients and their families, ensuring all care is provided with compassion, dignity, and respect.
· Undertake individual patient assessments, formulating goal setting and plans of care
· Be responsible for agreed service coordination and quality tasks, for example data collection, audits, attendance at multidisciplinary meetings.
· To lead creative therapies within the Wellbeing unit with support from volunteers and external services.
· Support and supervise the work of Support Workers, Students and Volunteers
· The post holder will provide individualised holistic enablement focused palliative rehabilitation within their scope of practice.
· Partake in the on-call rota.
· Contribute to and participate in organisational service development.
· Work across the clinical services, including Hospice at Home when required, to ensure service delivery.
