Job overview
An exciting opportunity has arisen within the Community based Multiple Sclerosis Specialist Nurse Team to appoint a Band 6 Multiple Sclerosis
Nurse. The base of the successful candidate will be determined on appointment
Working across Northumberland the post holder will work with the Specialist Multiple Sclerosis Nurse and MS Nurses to deliver a comprehensive specialist clinical service for people affected by Multiple Sclerosis (MS), with an emphasis on patient centred care and symptom management
The post holder will be expected to promote integrated care, working across services and Trusts to ensure quality of service and adherence to
local and national standards.
The post holder will be able to demonstrate the ability to act autonomously, where necessary, within a small team with the appropriate level of clinical supervision from the Specialist Nurse Team Manger
The post holder will be a First Level live NMC Registered Nurse with a professional degree or equivalent experience. They will have experience of co-ordinating care for people with long term conditions and/or multiple sclerosis in a variety of settings.
The post holder will be expected to attend a week long residential Development Module in Clinical MS and to attend further training as required.
Participation in the education and teaching of people affected by MS and Health and Social Care Professionals will be expected.
Main duties of the job
- To support the provision of a specialist nurse resource for individuals with Multiple Sclerosis, their relatives & carers, members of the
general public, health professionals and statutory & voluntary bodies. - Promotion of integrated care, ensuring quality of service and adherence to local and national standards.
- Ability to act autonomously within a small team with the appropriate level of clinical supervision from the specialist nurse team lead.

Detailed job description and main responsibilities
Deliver a community based, comprehensive, specialist clinical service for people affected by Multiple Sclerosis (MS), with an emphasis on patient
centred care and symptom management.
To provide people affected by MS and Health and Social Care Professionals with support, education, advice and information in order to manage the condition effectively
To provide a timely response to acute exacerbations of, or continued progression of MS and co-ordinate the appropriate level of care to enable
them to remain at home wherever possible
To assist the MS specialist nurse in the monitoring & review of patients in Northumberland who may be eligible for disease modifying treatments.
To assist in the facilitation and delivery of early supported discharge for people with MS.
Ensure accurate documentation and patient records are maintained in line with organisational policy & NMC standards.
Participate in regular audit/research evaluating practice and participate in local and national research projects.
To support the MS Specialist Nurse in the development and delivery of specialist training & education in MS to people affected by MS, health
professionals, allied health professionals, social care staff and students.

Qualifications
Essential criteria
- Professional registered nurse (Adult)
- 1st level degree or equivalent experience
- Specialist knowledge of long term conditions.
- Mentorship qualification Evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period unless post is bank only).
Experience and knowledge
Essential criteria
- Knowledge of multiple sclerosis, the condition and management of symptoms
- Knowledge of nursing models & evidence based care delivery i.e., NICE guidelines
Desirable criteria
- Knowledge of disease modifying treatments.
- Knowledge/understanding of change management and service development.
- Knowledge of the Trust values, vision and key priorities.
