Dementia Outreach Coordinator Job Description Post: Salary: Dementia Outreach Coordinator (Part-time) £21,840 (£27,300 FTE) Place of work: Clockhouse / Community Locations Hours of work: 28 hours per week, Monday, Tuesday, Wednesday & Thursday Reporting to: Dementia Service Lead Reporting to Jobholder: Volunteers Job Offer and Employment subject to satisfactory Right to Work Checks and Enhanced Disclosure & Barring Service (DBS) check. Job Purpose To support individuals living with dementia and their carers. Delivery of group activities e.g. carer support, dementia activity groups & Maintenance Cognitive Stimulation Therapy (MCST). Development and delivery of tailored 1-1 respite / sitting services / activity sessions in people’s own homes / community locations. Main Duties To include the following: • To plan and organise dementia activity groups in community locations • To deliver the MCST programme in community locations • To establish and run carer support groups in community locations • Connecting and developing a community network of partners to assist in facilitating groups • Work with volunteers to ensure the smooth running of group sessions • Taking payment for sessions and maintaining records of transaction August 2025 • Assessing individuals and their carers in the home environment and developing support plans to enable engagement and meaningful activity for the person living with dementia and respite for the carer • Delivering against agreed care plans to support individuals living with dementia on a 1-1 basis in their homes and at community locations Person Specification Skills and abilities • Able to organise and lead group meetings and activities • Able to work autonomously • Excellent communication and inter-personal skills • Excellent organisational skills and ability to determine priorities in a pressurised environment • Proficient in MS Office and Database Management • Adaptable, flexible and self-motivated and prepared to go the extra mile • Experience of delivering high quality customer service • Experience of working with people living with dementia and their families • Experience in adult safeguarding • Highly organised and able to develop and maintain volunteer and partner rotas • Caring, compassionate and patient • Confident driver Other requirements • Must be fully supportive of diversity and equality of opportunity in the workplace • Must be willing to undertake training relevant to the role and update this when required • Car driver with use of own vehicle for work purposes Desirable • Current DBS on update service, adult workforce Working conditions: • The nature of this post will require travel therefore a requirement of this post will be access to and the use of a vehicle with a current driving licence and business use insurance. • This post will involve some lone working therefore the advice and guidance of Age UK Surrey must be followed August 2025
