We have an exciting opportunity for an enthusiastic and caring Hospital to Home Assistant Coordinator to join our hard-working team at Gosport War Memorial Hospital, in Gosport. The role will provide short-term support, tailored to individual need, to help patients return home after their stay in hospital.

Working 30 hours per week, your responsibilities would include;

• Coordinating and managing the Hospital to Home Service for patients receiving care and rehabilitation in hospital working in partnership with various departments of the hospital.
• Engaging with patients to elicit and convey information that contributes to the Multi-Disciplinary Team (MDT) process and support decisions.
• Working with local community organisations to identify resources and support services for patients who are preparing for discharge home.
• Managing post-discharge patient contact with a view to offering ongoing community support if required.
• Support for H2H Volunteers to support patients with their transition from hospital to home and ensuring they are settled
• Volunteer Recruitment and support
