Project Description
The project operates Swansea working with women who have substance misuse issues and other complex needs, such as domestic abuse. The service provides residential support with the aim of meeting individuals’ needs, which promote independence and lead to a sustainable move-on. The project also provides housing related support within people’s homes or in the community, with the aim of preventing homelessness and increasing independence.
Purpose of Role
To provide residential and floating support that enables and empowers Service Users to address housing and/or personal difficulties, build their skills and confidence, and ultimately improve their ability to maintain their future or current accommodation. The support will not create or increase dependence. It will ‘do with’ rather than ‘do for’, so it’s likely that support will involve accompanying Service Users initially, in order to familiarise them with an activity or appointment. Support will help people improve their control, understanding and involvement in dealing with the issues that affect them, and assist Service Users to achieve their preferred outcomes. In addition, to assist with the housing management needs of the project ensuring the delivery of a safe, secure and effective service.
This role is subject to an Enhanced DBS disclosure.

Led by Spectrum Community Health CIC, Reconnected to Health is delivered by Spectrum, Tees, Esk and Wear Valleys NHS Trust and Waythrough to provide holistic, collaborative care to men and women in custody across the region. We provide drug and alcohol recovery co-ordination, psychosocial interventions, and continuity of care planning within the integrated prison healthcare system. We work with community services throughout the country and have excellent links with all Northeast community drug and alcohol services.
Reconnected to Health offers support to residents within a secure setting to recover from drug and/or alcohol dependence.
The aim of the Healthcare Navigation Reconnect Service is to reduce health related barriers that directly impact on the individual’s engagement prior to, and upon leaving prison; to reduce health inequalities and address the health and wellbeing of low to medium risk of offending behaviours. As an Reconnect Navigator, you will promote continuity of care support for those leaving prison, encouraging positive and sustainable connections for service users within their local communities and building recovery capital assets.

- NVQ Level 3 Health and Social Care or above or equivalent.

To deliver a high-quality service to and facilitate opportunities for individuals with spina bifida and/or hydrocephalus and their families/carers through Shine membership. SHINE has over 15,000 members with spina bifida and/or hydrocephalus. This post will bring a real focus to Cambridge and London, as well as the Essex, Suffolk & Norfolk area and will be an exciting opportunity to engage with members and develop and deliver the services we offer.
We work with members of all ages from our Little Stars aged between 0 – 12, young people are members of our FIRE project and we have established adult support. Working alongside our multi skilled, established team, you will be supporting members across all age groups and building on the great foundations already in place.
The role is home based but you will be required to attend clinic at Addenbrooke's Hospital Cambridge on the first Friday of every month. You may also be required to cover other clinics, events in London and meet members one to one where necessary within your allocated area of Essex, Suffolk & Norfolk, and occasionally across wider areas and nationally including meetings at our head office in Peterborough.
- To attend and represent Shine at Addenbrooke's Hospital clinic in Cambridge on the 1st Friday of every month and to cover other clinics where required.
- Delivering Shine’s welcome process for new members in the allocated area and nationally.
- To provide information to individuals, families, carers affected by spina bifida and/or hydrocephalus including hard to reach communities living within your area.
- To provide advice, support and guidance to enable people with spina bifida and/or hydrocephalus to access the correct benefits to become financially stable, the right services to improve their health and well-being, and to better co-ordinate their support and independence through a variety of methods. This will include attending specialist clinics in the area to engage with them.
- To refer individual members and/or their families/carers to the most appropriate source of expertise and support in Shine’s Health team and other designated leads within the organisation, or signpost to external agencies as appropriate.
- To develop age-specific groups and peer support networks which will provide opportunities for people with spina bifida and/or hydrocephalus to connect and interact, enabling social opportunities and to share experiences, support and learn from each other.
- To work with and develop opportunities for Shine volunteers.
- To contribute to the delivery of regional and national programme of events to Shine’s membership, based on identified need and interests, geographical location and age. These will be face to face and virtual events.
- To provide information to professionals working with those individuals and families affected by spina bifida and/or hydrocephalus.
- To raise greater awareness of spina bifida and/or hydrocephalus amongst professionals, external organisations and the general public through presentations, training and attendance at professional events.
- To actively promote membership of Shine to people with the relevant conditions.
- To keep clear and accurate records for monitoring and evaluation using our Salesforce database system.
- To work with other service, fundraising, marketing, trust and foundations colleagues and Shine’s membership to identify and seize potential fundraising opportunities for long-term sustainability of the work.

NYAS are looking to recruit, Self-Employed Social Workers to work alongside a solicitor in the NYAS tandem model, when the Court have appointed NYAS as Guardian for a child or young person in private family law proceedings, in circumstances where the parents cannot agree on what is in the best interest of their children, necessitating the child to have an independent voice and on welfare decisions about them. Listening to and hearing the children and their wishes and feelings.
The appointment of NYAS is under Rule 16.4 of the Family Procedure Rules 2010.
As a NYAS caseworker, you will be making recommendations to the Court on what you believe what decision the court should make, ensuring the welfare of the child is your paramount consideration in everything you do.
NYAS will expect you to be a qualified social worker, with no less than 5 years’ experience of children’s social care, registered with Social Work England.
You will complete reports and assessments, that will require you to work directly with children to understand their lived experiences, have appropriate skills to obtain a child’s individual wishes and feeling, taking into account any special needs the child may have, demonstrate strong analysis of risk to ensure the children’s safety, and articulate your findings and recommendations in a high-quality assessment. Consulting where necessary with a other professionals to inform that assessment.

We are looking to grow our team of dedicated, caring, enthusiastic and friendly support workers to work with three ladies in the Bedlington and Ashington areas.
We support two ladies with learning disabilities who share a bungalow in Bedlington. One lady requires a lot of personal care support, while the other requires minimum amount. They both receive 1:1 support therefore for the majority of each day there are two staff members on shift. From 8pm each night there is one person on shift which includes sleep over support. Each of these ladies have their own different hobbies. Both ladies like to access the local community on a daily basis and go out for tea and coffee. One lady loves to collect items and enjoys annual holidays in the UK.
We also provide enabling support (25 hours per week) for one lady who lives in Ashington. We support her with her finances, help with weekly shopping, meal planning and cooking. One lady loves shopping, visiting charity shops and socialising.
What we're looking for:

About the role
The role of the Support Worker is to provide specialist expertise and support to facilitate teams in achieving person centred outcomes for the people we support, some of whom have profound and complex needs, to maximise the individual’s independence and enable them to be active in their communities.
Duties will include:
- Making a difference in the lives of those you support
- Help to use a range of recovery-oriented and person-centered planning tools
- Ensure all plans and records relating to the people we support are accurate and up to date
- Support with everyday tasks to maximise the individual’s independence
- Work in a consistent and positive way with local managers and other team members to ensure people get a great service
- Undertake various levels of personal care and manual handling
About you
To be a Support Worker at Certitude, it is essential you have experience for this role in:
- Supporting people who may challenge (verbally, physically) particularly people on the autistic spectrum
- Keeping calm and confident in challenging situations
- Experience in Positive Behaviour Support
- Experience of enabling people with a learning disability/autism to take positive risks in their lives
- Possess a manual driving license and be able to drive a 7-seater van
- Knowledge of the Transforming Care Agenda
You will be given training, development and coaching to help you feel confident in providing great support and to help you advance in your career.

Specialist Community Social Worker
£34k – 40k (Pro Rata and subject to experience)
Part-time 21.6 hours per week.
Full time annual leave entitlement is 28 days.
The newly created Community Social Worker will play a vital role in supporting adults and improving the lives of individuals, families, and groups from within the Deaf, hard of hearing, deafened, Deafblind community

A strength based approach will underpin your approach to problem solving to help people navigate through challenges and find person led interventions and strategies.
You are committed and able to clearly demonstrate the professional standards of social work, from theory to application using up to date best practices.
Joining a highly supportive and compassionate social work team diverse in their different skills and strengths, you will be self-motivated and equally work well as part of a team.
Culturally aware you will have an avid interest and understanding of the need of Deaf, hard of hearing, deafened, Deafblind community. Be passionate about promoting accessible services for those in Surrey.
Excellent communication and IT skills, the ability to work flexibly, collaborate with others, and have an enthusiastic ‘can do’ approach are essential.
You must be a Registered Qualified Social Worker with an up-to-date registration. We welcome applications from the Deaf Community who can confidently communicate in British Sign Language. If you are not fluent in BSL (level 2 min) yet bring other strengths, we will support your learning to excel in your role and access interpreters.

At Turning Point, we support people across The UK with Mental Health, Substance Misuse and Learning Disabilities. As a Youth Support Worker, you will ensure that young people living in Turning Point’s supported accommodation are supported to thrive and reach their full potential in a safe, nurturing, and supportive environment, increasing their independency to create a positive move on into the community.
As a Youth Support Worker, you will join The Lancashire Young People supported accommodation services that supports 18 young people (Aged 16- 21) at our service in Leyland, The Mill.
Role Responsibility
You will be supporting Young People to manage their finances, gain independent living skills, promote health, wellbeing and healthy lifestyle choices as well as supporting them to maximise their involvement in local training, education, and employment opportunities.
The role is changeable, and every day will be different, some examples of what you will be responsible for are:
- Managing the referrals, sign up, support and move on for the Young People
- Developing, implementing and reviewing individual support plans and risk assessments focussing on young people’s needs, strengths and goals.
- Providing support with budgets and finances
- Provide an excellent rent / service charge collection service, ensuring regular accounts monitoring and contact with people regarding payments
- Completing accurate and detailed daily records for the Young People
- Providing practical support and guidance to the Young People including daily living skills, cooking, cleaning, and ‘moving on’ plans
- Ensuring proactive and constructive communication to the Young People including facilitating weekly key-working sessions, problem solving and supporting with their day-to-day needs
- Supporting the Young People and landlords with housing management tasks addressing practical aspects of living in supported housing, including managing repairs, tenancies, income and anti-social behaviour.
- Prioritising your own case load and using your initiative to plan and implement agreed actions
- Building and maintaining relationships with a range of other professionals to work together to aim for positive outcomes for the Young People
- Facilitating and arranging engaging activities with tenants throughout the year.
- To work alongside colleagues, and management in shaping and developing the way we provide and manage our schemes, including improvements to processes and how we work with young people
- Following organisation policies and procedures to ensure compliance of the service
- Turning Point responds to the needs of the individuals and some of the tasks are therefore unpredictable. It is expected that the Youth Support Worker will work in a flexible way when required and undertake tasks that are not specifically covered within the job description, which can include cover for colleagues as required.
The Ideal Candidate
As a Youth Support Worker, we are looking for candidates who have knowledge and experience of working with young people with complex needs, homelessness, children who are looked after and in transition to leaving care or care leavers.
We’re ideally looking for a person with a NVQ level 3 or equivalent in Social Care, Youth Support Work or Housing/Homelessness, however this is not essential for the role. We are also looking for people with a background of working with young people and an understanding of homelessness and the impact this has on the wellbeing of Young People.
Ideally you will have experience in support planning, re-engaging and motivating young people to participate in employment and work programmes, education, training, volunteering, and skill building opportunities.
You need to be a great team player with lots of energy, you’ll need strong communication skills, a flexible approach and positive attitude to achieve targets in a challenging environment which requires empathy, understanding and patience.
Shift patterns
