Location of Post:
The Young Person Wellbeing Practitioner will be based from the Westbourne Centre in Rhyl and will supporting young people in local schools/community hubs and family homes. A digital offer will also be made available to the service users.
Summary of Service:
Service 2- Young Person's Mental Health and Emotional Wellbeing Service.
Aim: To offer 1:1 and/or group support to young people to improve their emotional wellbeing through tailored support packages which is reviewed on a regular basis to ensure their voice is heard in the progress of their plan- to help them reach their desired outcomes.
Who is it for: Young People aged 12- up until their 18th Birthday (depending on assessed need).
Job purpose
The post holder will directly support young people to positively manage emotional well-being issues.
Job Activities:
To meet with the young person and support with their emotional health and wellbeing needs on a regular and planned basis. An assessment of their needs will be required to help formulate a support plan.
- To facilitate communication and engagement with young people using a range of approaches.

Cove Parish
We are a thriving church family with a heart to reach out to our wider community, demonstrating gracious generosity, and whole life worship whilst reflecting Jesus, inspiring hope and loving joyfully. Cove in Farnborough, North Hampshire is geographically and numerically a large, diverse, parish with 8, soon to be 9 schools, expensive housing developments together with estates with a high amount of social housing. We are a 5 minute drive from Farnborough town centre with shops, restaurants and a cinema and the parish is blessed with large green spaces, wooded areas and several country parks.
We are one family and are privileged to have two church buildings (a smaller Victorian church and a more flexible building). Our two churches and parish are hives of activity all week hosting many outreach missional initiatives; Bumps and Babies and Little Fishes welcome and support new and young families. Open Door, Meeting Point, both Footprints cafes, and Cove Community lunch are a haven for those struggling with loneliness, the cold and hunger. With courageous faith we recently took over Farnborough Foodbank recognising the need within our town as it supports over 100 families every month. We regularly holds assemblies in schools, often accompanied by the Open the Book team. In turn the schools visit our church buildings for Christmas and Easter services, pay tribute on Remembrance Sunday as well as take part in our creative outreach initiatives like the Christmas Journey and Easter Workshops.

This new role will put you at the heart of an innovative partnership across West Yorkshire that is changing how we respond to stalking. As part of the Safer Steps Team, you will provide specialist support to high-risk, vulnerable children and young people who are experiencing stalking, whether it is domestic abuse-related or non domestic abuse-related. Your work will help shape a safer, more coordinated approach to rotecting young people and improving their long-term wellbeing.
You will play a key role in raising awareness and understanding of stalking and its unique impact on young victim-survivors across services and partner organisations. Through your work, you will help break down barriers to support, ensuring children and young people can access the help they need when they need it most. You will also contribute to improving processes and pathways, helping to shape a more responsive and coordinated approach for both current and future victim-survivors.
We’re looking for someone who is empathetic, compassionate, and committed to making a real difference in the lives of vulnerable young people. You’ll have experience working within a partnership or multi-agency environment, building trust, and supporting others through challenging circumstances.

Are you a proactive social worker looking to break the mould? This is not your standard statutory role. You will be joining OneBexley, a pioneering consortium of eight local charities that has been working together to deliver Care Act support in a completely new way for the past five years.

The role
We are growing our team and looking for a third social worker to join us. You will play a pivotal role in maintaining the high standards of this innovative model by mentoring our team of trusted assessors.
Your key responsibilities
- Empower the team:Provide regular supervision to trusted assessors, ensuring their wellbeing and career growth.
- Quality assurance:Sign off and authorise Care Act assessments and reviews produced by the team.
- Expert guidance:Act as a source of professional advice, supporting staff with difficult conversations regarding safeguarding or financial contributions.
- Case management:Oversee the triaging and allocation of referrals to ensure they align with our expertise.
