About the Role
As Recovery Coordinator you will work as part of a team to provide collaborative recovery planning and case management to substance misusers (drug and alcohol) across all stages of individuals’ recovery journeys, from assessment and engagement through to sustained recovery.
What You’ll Do
- Effectively manage a caseload of service users, reviewing and updating on-going processes such as risk assessments, risk management plans and safeguarding assessments and plans.
- Work closely and collaboratively within the multi-disciplinary team across the service and externally, providing advice and reports/ information to managers and other colleagues, through attendance at reviews, team meetings, supervision and via telephone conversations.
- Improve outcomes for children, parents/carers and families of services users by reducing the impact of drug and alcohol related harm on family life and promote positive family involvement in recovery.
- Develop and deliver quality programs and packages of work and psychosocial interventions that meet local need across both group work and one to one formats.
- Identify and prevent potential harm to service users and others by following local and organisational safeguarding guidelines, including referral to and engagement with relevant safeguarding authorities.
- Support delivery of key service functions through actively participating in drug screening, duty work, access to BBV testing and immunisation service provision, community needle exchange services and the ongoing delivery of appropriate, tailored alcohol and drug harm reduction interventions.
To Succeed in This Role, You’ll Need:

The Role
An exciting opportunity has arisen for a Recovery Coordinator to support Waythrough’s service delivery within Reconnected to Health in HMP Holme House . As a Recovery Coordinator, you will provide 1:1 support for service users, from brief interventions through to structured treatment, including groups, to ensure that every recovery journey is tailored to individual priorities. You will take a “whole person” holistic approach to ensure that every service user’s package of care encompasses all areas of their wellbeing, to achieve the best possible outcomes for every person.
- Manage a service user caseload, devising, implementing and reviewing on-going recovery plans and treatment, liaising closely with the full range of workers and skills within the service team, including family members and significant others as appropriate.
- Carry out triage/screening assessments and comprehensive assessments with service users.
- Work collaboratively with partner agencies to undertake shared assessments, key working and interventions to promote individual service user progression from engagement, through structured treatment to self-sustained recovery.
- Facilitate service user access to treatment and community resources to increase recovery and social capital, supporting service users and their families within their local community To lead effective case management for allocated service users, including on-going processes such as risk assessments, risk management plans and safeguarding assessments and plans. About the CW+ Arts in Health programme Our pioneering Arts in Health programme focuses on the link between human health and the environment, building on the robust body of evidence that outstanding design and engagement with the arts can improve physical and mental wellbeing. We hold a collection of more than 2,000 works of art and digital installations, many of which are on display around our hospitals for all to enjoy. And our Arts for All programme brings daily music, performance and creative activities into the wards, using art as a tool for healing. Our Design and Environment programme works with renowned partners who specialise in health, technology and environmental design, to address the factors which create the optimum healing environment. Become an Art and Design Research Volunteer This is an exciting opportunity to support the research and evaluation of the Arts in Health programme. We are looking for an individual with an interest in arts in health, design, and research or evaluation methods, who is looking to gain experience in data collection and analysis in a busy healthcare setting. As a Positive Behaviour Support Partner you will believe in our ethos and values, and you will want to make a difference in someone’s life. The primary purpose of the PBS team is to lead on the day to day delivery of Positive Behaviour Support within the service, including the creation, continuous development and implementation of Positive Behavioural Support Plans. This will be carried out through regular assessment and observations of the people we support.
The PBS team work closely with staff teams to provide a consistent person centred approach, to share good practice, advise and upskill in the principles of PBS.

Your role will be to support people returning to their community following a short-term sentence or period on remand providing relational and practical support that is tailored and responsive to the needs of the person. Working collaboratively with Throughcare Prison Worker colleagues and a range of partners you will support in co-ordinated pre-release planning to ensure a person's journey is effectively supported. This will involve supporting connections to local, and specialist, services where appropriate and importantly wider community connections.

You will be self-motivated and understand the importance of collaboration and working in partnership across organisations and sectors to support the best outcomes for people. Rainbow Trust Children’s Charity enables families who have a child with a life threatening or terminal illness to make the most of time together, providing expert, practical and emotional support, where they need it for as long as it is needed. We deliver a range of MOJ Commissioned Rehabilitative services (CRS) directly for Probation in three key areas: Accommodation, Personal Wellbeing and Dependency & Recovery. Accommodation is delivered in Wales, Dependency and Recovery in London and Humberside, Personal Wellbeing in East of England, Sussex, Surrey and as a subcontractor in Kent and Cheshire.
These services work with Men 18+ who are under probation supervision, supporting them to navigate through their desired outcomes in order to achieve transformational change, break the cycle of offending and achieve long-term integration into society.

Roles Responsibilities
As a Recovery Navigator in the Dependency and Recovery Services, you will be working within a hybrid model as part of a flexible, multi-disciplinary team, working in person and travelling across the probation delivery units which covers the region of: Southwark, Croydon, Lambeth. You will be based in Borough, Croydon, Brixton probation offices. Some work may be delivered virtually, however this is an in-person service.
The service is designed to improve engagement of Person(s) on Probation into treatment services and ‘bridge the gap’ between probation and substance misuse services in the community by offering practical assistance and ongoing support through a range of timebound, enforceable and motivational interventions.
You will carry a diverse caseload of adult male prison leavers and those on community orders within the National Probation Service, supporting to implement their action plan and navigate their recovery journey. You will be responsible for completing comprehensive assessments, creating a collaborative action plan and delivering one to one, group work/virtual sessions, whilst maintaining accurate record keeping throughout.
Your role is to successfully navigate these men on their recovery journey by providing:
- Support and advocacy e.g. including physical help with referral forms, face-to-face negotiations with a wide range of other providers, attending appointments with them or taking steps on their behalf, for example making phone-calls and onward referrals.
- Advice, guidance and information e.g. Information via a variety of forms (including printed documents, one to one session, group session, online session or a combination of any of the above) but must be provided in a way that enables the Person(s) on Probation to act on the information given and should form part of a wider package of support offered.
- Deliver interventions to empower and upskill Person(s) on Probation to enable understanding and management of their Dependency and Recovery needs.
Some degree of flexibility may be required if out of office work is required, but this is a predominantly a Monday - Friday 09:00-17:00 role.
All probation-based roles will require enhanced DBS and HMPPS security vetting. Please note this process can take up to 3-6 weeks. All offers are subject to receiving both HMPPS vetting and DBS clearances.

Working in the HOPE team is an exciting and varied role that combines outreach, community engagement, and harm reduction. The team works across the city, building relationships with individuals and communities who may be difficult to engage. The role involves providing practical support through the needle and syringe programme, working alongside people with complex needs, and promoting safer practices. It also includes delivering training, attending events and working closely with partner organisations to raise awareness of the service and ensure people can access the right support at the right time.

We are looking for a dedicated, passionate and skilled Recovery Support Worker to join our new team at Beach House. The service provides high support housing to service users with enduring mental health backgrounds. The service consists of 8 flats and offers a full recovery and rehabilitation package to our service users.
We aim to support and assess on-going needs of our service users to build on their existing skills while learning new ones. All service users are allocated to a named Support Worker. Service users are required to fully participate in the process of planning their care, which includes attending support/key work sessions with staff; this time is for clients to discuss any issues or areas of concern they may have, or perhaps go out for a coffee/trip to local activities, or be supported with a practical task in their flat such as cleaning or laundry.
Role Responsibility
You will provide recovery focused support to service users in accordance with their support plans. You will have well-honed skills in positive communication, engagement, and will be able to demonstrate unconditional positive regard for service users.
You will have the maturity to work within a positive framework supporting clients. You will have the ability to deliver services and interventions in a person centred, non-judgmental manner, and recovery-based way.
You will work alongside members of the service user’s circle of support, to enable individuals to be happy, healthy, and successful in achieving their life goals.
